AHRQ Grantee Profile: Brian Jack, M.D.

AHRQ’s latest grantee profile features Brian Jack, M.D., who developed the Re-Engineered Discharge (RED) protocol that uses strategies to promote safer patient care and reduce avoidable hospital readmissions.

Jack, a professor of family medicine at Boston University School of Medicine, lowered rates of return trips to the hospital. AHRQ also worked with him to develop an implementation-ready toolkit and guide for patients (in English and Spanish) so health providers can apply RED and address language barriers in health care communications.
