External validation of the STONE score, a clinical prediction rule for ureteral stone: an observational multi-institutional study.
The STONE score is a clinical decision rule that classifies patients with suspected nephrolithiasis into low-, moderate-, and high-score groups, with corresponding probabilities of ureteral stone. The researchers evaluated the STONE score compared with physician gestalt. They concluded that in its present form, the STONE score lacks sufficient accuracy to allow clinicians to defer CT scan for suspected ureteral stone.

Osteoporosis overtreatment in a regional health care system.
The researchers estimated the frequency of osteoporosis overtreatment in a regional health care system where DXA reports routinely include T scores for anatomic sites (e.g., lateral lumbar spine) that the International Society for Clinical Densitometry does not recommend for osteoporosis diagnosis. They found that two-thirds of new osteoporosis drug prescriptions were potentially inappropriate.

Development and internal validation of a model for early detection of hepatocellular carcinoma in patients with cirrhosis.
The authors sought to derive and validate a model to accurately distinguish cirrhotic patients with and without hepatocellular carcinoma (HCC) and compare the accuracy of the model to that of α-fetoprotein (AFP) alone. They found that models including AFP and FIB-4 can accurately discriminate cirrhotic patients with early-stage HCC from those without HCC.

Appropriateness of cardiac stress test use among primary care physicians and cardiologists in the United States.
In this study, the researchers used nationally-representative data to examine differences in appropriate use of cardiac stress testing between cardiologists and primary care physicians (PCPs) in the US. Using permissive criteria to determine appropriateness, they found that primary care physicians are more likely to order or perform a rarely appropriate cardiac stress test than cardiologists.

Investigating the utility of previously developed prediction scores in acute ischemic stroke patients in the stroke belt.
The researchers compared several scoring systems among patients receiving systemic and endovascular treatments. They concluded that their study demonstrated that although highly predictive of outcome in the original study design treatment groups, prediction scores may not generalize to all patient samples, highlighting the importance of validating prediction scores in diverse samples.

Role of body temperature in diagnosing bacterial infection in nursing home residents.
The goals of this study were to provide empirically based guidance on the role of body temperature and fever in the clinical diagnosis of infections. When the researchers compared their empirically derived definitions of fever with those commonly used to justify the prescription of antibiotics for systemic infections, they concluded that the empirical, less-stringent definitions appeared to better fit the clinical realities of nursing home work.
