Screening performance of digital breast tomosynthesis vs digital mammography in community practice by patient age, screening round, and breast density.
The purpose of this study was to compare digital mammography (DM) vs digital breast tomosynthesis (DBT) performance by age, baseline vs subsequent screening round, and breast density category. Information was taken from screening examinations at participating Breast Cancer Surveillance Consortium facilities of 1.5 million women aged 40 to 79 with no prior history of breast cancer. Findings showed that improvements in recall and cancer detection rates with DBT were greatest on baseline mammograms. On subsequent screening mammograms, the benefits of DBT varied by age and breast density, and women with extremely dense breasts did not benefit from improved recall or cancer detection with DBT on subsequent screening rounds.

Clinical exome sequencing vs. usual care for hereditary colorectal cancer diagnosis: a pilot comparative effectiveness study.
The purpose of this study was to evaluate clinical exome sequencing (CES) compared to usual care (UC) in the diagnostic work-up of inherited colorectal cancer/polyposis (CRCP) in a randomized controlled trial (RCT). The investigators indicate that their results suggest that CES provides similar clinical benefits to multi-gene panels in the diagnosis of hereditary CRCP.

Performance characteristics of digital vs film screen mammography in community practice.
In this study, the investigators examined performance characteristics of digital vs film screen mammography in community practice by comparing the performance characteristics of 297 629 full field digital (FFDM) and 416 791 screen film mammograms (SFM). In their study, modest gains in performance were achieved with the introduction of FFDM as a replacement for SFM, in a large health care organization.

Clinical correlates of acute bipolar depressive episode with psychosis.
The aim of this study was to identify the characteristics of depressed bipolar patients with current psychosis compared to those without psychosis. Results showed that psychosis was present in 10.6% of the studied depressed patients. Psychotic patients had less education, lower income, and were more frequently single and unemployed. Psychosis was also associated with a more severe depressive episode, higher suicidality, more comorbid conditions and worse functioning. Most group differences disappeared when controlling for depression severity.

Comparison of comorbidity scores in predicting surgical outcomes.
The purpose of this study was to compare diagnosis-based and prescription-based comorbidity scores for predicting surgical outcomes. It concluded that the Centers for Medicare and Medicaid Services-Hierarchical Condition Categories had superior performance in predicting surgical outcomes. Prescription-based scores, alone or in addition to diagnosis-based scores, were not better than any diagnosis-based scoring system.

Diagnostic performance of cardiac magnetic resonance imaging and echocardiography in evaluation of cardiac and paracardiac masses.
The researchers evaluated the predictive value of echocardiography and cardiac magnetic resonance (CMR) imaging parameters to identify tumors and malignancy and to provide histopathologic diagnosis of cardiac masses. They found that CMR identified 6 masses missed on transthoracic echocardiography (4 of which were outside the heart) and provided significantly more correct histopathologic diagnoses compared to echocardiography (77 percent vs 43 percent).

Urinary biomarkers for diagnosis of bladder cancer: a systematic review and meta-analysis.
The authors systematically reviewed the evidence on the accuracy of urinary biomarkers for diagnosis of bladder cancer in adults who have signs or symptoms of the disease or are undergoing surveillance for recurrent disease. Their review concluded that urinary biomarkers miss a substantial proportion of patients with bladder cancer and are subject to false-positive results in others.

Diagnostic methods for myalgic encephalomyelitis/chronic fatigue syndrome: a systematic review for a National Institutes of Health Pathways to Prevention Workshop.
The purpose of this systematic review was to evaluate and compare studies of methods to diagnose myalgic encephalomyelitis /chronic fatigue syndrome (ME/CFS), identify limitations of current studies, and determine needs for future research. It found that 9 sets of clinical criteria are used to define ME/CFS, yet none of the current diagnostic methods have been adequately tested to identify patients with ME/CFS when diagnostic uncertainty exists.

Imaging techniques for the diagnosis of hepatocellular carcinoma: a systematic review and meta-analysis.
This review evaluated the test performance of ultrasonography, computed tomography (CT), and magnetic resonance imaging (MRI) in the detection of hepatocellular carcinoma (HCC). It concluded that CT and MRI are associated with higher sensitivity than ultrasonography without contrast for detection of HCC. For detection of focal liver lesions, the sensitivities of all three techniques are similar.

Endometrial cancer after endometrial ablation vs medical management of abnormal uterine bleeding.
The authors investigated whether endometrial ablation is associated with increased risk or delayed diagnosis of endometrial cancer compared with medical management of abnormal uterine bleeding. They observed no difference in endometrial cancer rates, nor was there a delay in diagnosis when comparing endometrial ablation vs medical management.

Survey of the methods and reporting practices in published meta-analyses of test performance: 1987 to 2009.
The researchers performed a survey of meta-analyses of test performance to describe the evolution in their methods and reporting. They found that the most common clinical areas were cardiovascular disease and oncology, while the most common test categories were imaging and biomarker tests. They concluded that the methods employed in meta-analyses of test performance have improved with the introduction of quality assessment checklists and the development of more sophisticated statistical methods.
