Quality of care improves for patients with diabetes in Medicare shared savings accountable care organizations: organizational characteristics associated with performance.
This study analyzed secondary data retrospectively to examine Medicare Shared Savings Program (MSSP) Accountable Care Organizations’ performance on diabetes metrics in the first 2 years of ACO contracts in order to determine how ACO organizational characteristics - such as composition, staffing, care management, and experiences with health reform - were associated with quality of care delivered to patients with diabetes.

The impact of dementia special care units on quality of care: an instrumental variables analysis.
The purpose of this study was to compare the quality of care following admission to a nursing home (NH) with and without a dementia special care unit (SCU) for residents with dementia. The investigators found that facilities with an SCU provided better quality of care as measured by several validated quality indicators. The authors suggested that given the aging population, policies to promote the expansion and use of dementia SCUs may be warranted.

Workforce configurations to provide high-quality, comprehensive primary care: a mixed-method exploration of staffing for four types of primary care practices.
The aim of this study was to explore the team configurations and associated costs required to deliver high-quality, comprehensive primary care. The study estimates provided health care decision-makers with needed guideposts for considering primary care staffing and financing and informed broader discussions on primary care innovations and the necessary resources to provide high-quality, comprehensive primary care in the USA.

Mind the overlap: how system problems contribute to cognitive failure and diagnostic errors.
This study focused on how system problems within two academic institutions contribute to cognitive and diagnostic errors of inpatient physicians. Observations were conducted by physicians, nurses, and non-clinicians (qualitative researchers, social scientists and health care engineers). Focus groups were also conducted. System-based problems included interruptions, time constraints and physical space.

Quality through coopetition: an empiric approach to measure population outcomes for emergency care-sensitive conditions.
Researchers developed a novel approach for measuring regional outcomes for emergency care-sensitive conditions. They identified regional use patterns for emergency conditions requiring a community-wide system response. They concluded that this method of attribution allows regional performance to be benchmarked and could be used to develop population-based outcome measures after life-threatening illness and injury.

The politics of primary care expansion: lessons from cancer survivorship and substance abuse.
The purpose of this study was to understand the perspectives of primary care innovators treating patient populations not traditionally considered to be within the purview of primary care. The authors indicated that their study findings suggested that the politics surrounding entrenched professional identities contributed to barriers faced by conference participants in their efforts to provide innovative care for these nontraditional populations. Specifically, obstacles surfaced in relation to sharing patients across disciplinary boundaries, which resulted in issues of possessiveness, a questioning of provider qualifications, and a lack of interprofessional trust.

Creating individual surgeon performance assessments in a statewide hospital surgical quality improvement collaborative.
In this study, the investigators sought to create surgeon-level comparative assessments within the Illinois Surgical Quality Improvement Collaborative. The investigators found that few individual surgeon performance outliers could be detected in NSQIP clinical registry data for a statewide hospital collaborative over a 30-month period using postoperative patient outcomes. The authors suggest that low surgeon-specific case volumes and minimal variance between surgeons may limit the utility of American College of Surgeons NSQIP outcomes measures for individual profiling.

Does participation in health information exchange improve hospital efficiency?.
This study strives to answer the question: does health information exchange (HIE) network participation improve hospital efficiency? The results of the study suggest that hospital investment in HIE participation may be a useful strategy to improve hospital operational performance, and that policy should continue to support increased participation and use of HIE.

Frequent hub-spoke contact is associated with improved spoke hospital performance: results from the Massachusetts General Hospital Telestroke Network.
This study investigated the association of a strong hub-spoke hospital connection with improved spoke hospital performance for acute ischemic stroke patients associated with the Massachusetts General Hospital Telestroke Network. Investigators identified 375 patients treated with tPA by conventional or telestroke methods from 2006-2015 with 16 spoke hospitals. There was a positive association between more frequent contact between a telestroke spoke and its hub and faster tPA delivery for patients.

Nurse practitioner practice environments in primary care and quality of care for chronic diseases.
In this study, the authors investigated the relationship between nurse practitioner (NP) practice environments and quality of care for chronic diseases. The investigators found that NP practice environment affected the quality of care for 2 chronic conditions. They assert that efforts should be implemented to improve NP practice environment to potentially improve care quality.

Associations of CAHPS composites with global ratings of the doctor vary by Medicare beneficiaries' health status.
This study examines whether the association of care coordination with global ratings of one's personal doctor varies by number of chronic conditions and self-rated health. Results were consistent with the hypothesis that patients in worse health weigh care coordination more heavily in global physician assessments than patients in better health. Emphasis on improving care coordination, especially for patients in poorer health, may improve patients' overall assessments of their providers.

Follow-up and time to treatment in an urban cohort of children with sleep-disordered breathing.
The purpose of this study was to evaluate follow-up and timing of sleep-disordered breathing diagnosis and treatment in urban children referred from primary care. Researchers found that half of the children referred for sleep-disordered breathing evaluation are lost to follow-up from primary care. Obstructive sleep apnea severity did not predict follow-up or timeliness of treatment. They conclude that these findings suggest social determinants may pose barriers to care in addition to the clinical burden of sleep-disordered breathing.

Medicare accountable care organizations of diverse structures achieve comparable quality and cost performance.
The purpose of this study was to examine whether an empirically derived taxonomy of Accountable Care Organizations (ACOs) was associated with quality and spending performance among patients of ACOs in the Medicare Shared Savings Program (MSSP). The investigators found that there was greater heterogeneity within ACO types than between ACO types; there were no consistent differences in quality by ACO type, nor were there differences in likelihood of achieving savings or overall spending per-person-year; and was evidence for higher spending on physician services for physician-led ACOs.

Variability in determining sepsis time zero and bundle compliance rates for the Centers for Medicare and Medicaid services SEP-1 measure.
This study compared sepsis ‘time zero’ and CMS SEP-1 pass rates among 3 abstractors in 3 hospitals. Abstractors agreed on time zero in 29 out of 80 cases, and the perceived pass rates ranged from 9 to 19 out of 80 cases (23%). The authors conclude that this variability in time zero and perceived pass rates limits the utility of SEP-1 for measuring quality.

Understanding the impact of variations in measurement period reporting for electronic clinical quality measures.
The purpose of the study was to understand the impact of varying measurement period on the calculation of electronic Clinical Quality Measures (eCQMs). Variations in measurement periods were associated with variation in performance between clinics for 3 of the 4 eCQMs, but did not have significant differences when calculated within clinics. Variations from standard measurement periods may reflect poor data quality and accuracy.

Improving quality in the care of patients with inflammatory bowel diseases.
In this article, the authors reviewed the history of quality initiatives in healthcare and identified ongoing gaps in inflammatory bowel disease (IBD) care with a review of current IBD improvement efforts taking place. Several targets for improving IBD care quality while moving forward into the 21st century were also identified.

Analysis of errors in dictated clinical documents assisted by speech recognition software and professional transcriptionists.
The purpose of this study was to identify and analyze errors at each stage of the speech recognition (SR) assisted dictation process. The study concluded that seven in 100 words in SR-generated documents contain errors; many errors involve clinical information. That most errors were corrected before notes were signed demonstrates the importance of manual review, quality assurance, and auditing.

Development and validation of the Pediatric Medical Complexity Algorithm (PMCA) Version 3.0.
This study’s objective was to modify the Pediatric Medical Complexity Algorithm (PMCA) to include both ICD-9-CM and ICD-10-CM code revisions as well to assess the sensitivity and specificity of the new PMCA version 3.0. The new version 3.0 was applied to data for children from the Seattle Children’s Hospital emergency department, day surgery, and/or inpatient encounter from January 2016 to June 2017. A blinded random sample of 300 children was used from 3 different classifications: those with chronic complex disease, children with noncomplex chronic disease, and no chronic disease. Sensitivity and specificity was rated as over 65% up to 93% for all classification types.

Quality of care for children with medical complexity: an analysis of continuity of care as a potential quality indicator.
The objective of this study was to examine the relationship between continuity of care for children with medical complexity (CMC) and emergency department (ED) utilization, care coordination quality, and family effects related to care coordination. The investigators measured ED utilization and primary care continuity with the Bice-Boxerman continuity of care index for 1477 CMC using administrative data from Minnesota and Washington state Medicaid agencies. They concluded that continuity of care holds promise as a quality measure for CMC because of its association with lower ED utilization and more frequent receipt of care coordination.

The development of heuristics for evaluation of dashboard visualizations.
Heuristic evaluation is used in human-computer interaction studies to assess the usability of information systems. This article develops a heuristic evaluation checklist that can be used to evaluate systems that produce information visualizations. The authors suggest that a checklist of usability heuristics for evaluating information visualization systems can contribute to assuring high quality in electronic data systems developed for health care.

Validation of new care coordination quality measures for children with medical complexity.
The purpose of this paper was to validate new caregiver-reported quality measures assessing care coordination services for children with medical complexity (CMC). Results showed that 19 newly-developed Family Experiences with Coordination of Care quality measures demonstrated convergent validity with previously-validated CAHPS measures. These new measures are valid for assessing the quality of care coordination services provided to CMC and may be useful for evaluating new models of care focused on improving these services.

A national report of nursing home quality and information technology: two-year trends.
The authors sought to answer these two research questions: What are the trends in information technology (IT) adoption in US nursing home facilities over 2 years? How are 2-year trends in IT adoption in US nursing homes related to nationally reported quality measures (QMs)? Using surveys of nursing home administrators and data from Nursing Home Compare, they concluded that multiple dimensions of IT sophistication influence QMs in every health care domain, providing an opportunity to design a reporting system that joins these important variables to be assessed on a national scale.
