Latino adolescent-mother language concordance, neighborhood deprivation, and vaccinations in community health centers.
Researchers examined the association between adolescent-mother language concordance and neighborhood social deprivation with adolescent vaccination completion in order to understand social/family factors related to Latino adolescent vaccination. They used a multistate, EHR-based dataset of community health center patients to compare three Latino groups; their sample included over 56,000 adolescent-mother dyads. Their findings indicated that English-preferring adolescents with Spanish-preferring mothers had the highest completion rates. The researchers concluded that future studies could attempt to understand what advantage this language dyad may have in adolescent vaccination completion.

Safety and immunogenicity of live viral vaccines in a multicenter cohort of pediatric transplant recipients.
This study’s objective was to determine the safety and immunogenicity of live vaccines in pediatric liver and kidney transplant recipients. This cohort study included 281 children, of which 270 had received a liver transplant, 9 a kidney transplant, and 2 were liver-kidney transplant recipients. None of the children included had completed their primary measles-mumps-rubella (MMR) and varicella-zoster virus (VZV) vaccine series and/or had displayed nonprotective serum antibody levels at enrollment between January 1, 2002, and February 28, 2023. Safety data was collected after these transplant recipients had received 1 to 3 doses or MMR vaccine and/or 1 to 3 doses of VZV vaccine. The median time from transplant to enrollment was 6.3 years, with the median age at first posttransplant vaccine 8.9 years. The majority of children developed protective antibodies following vaccination (107 of 149 varicella, 130 of 152 measles, 100 of 120 mumps, and 124 of 125 rubella). One year post vaccination, the majority of children who initially mounted protective antibodies maintained this protection (34 of 44 varicella, 45 of 49 measles, 35 of 42 mumps, 51 of 54 rubella). Five children developed varicella, all of which resolved within 1 week, and there were no cases of measles or rubella and no episodes or graft rejection within 1 month of vaccination. There was also no association between antibody response and immunosuppression level at the time of vaccination.

Effectiveness and feasibility of three types of parent reminders to increase adolescent human papillomavirus (HPV) vaccination.
Efforts to boost human papillomavirus (HPV) vaccination rates among adolescents have seen moderate success with the use of parent notifications. Nonetheless, there is limited research on the comparative efficacy and practicality of various HPV reminder methods in settings with restricted resources. The purpose of this quasi-experimental study (2016-2017) assessed the impact of three types of parent reminders (written correspondence, automated phone calls, and SMS messages) on HPV vaccine uptake for the subsequent dose among 12-year-olds at a prominent Federally Qualified Health Center in Los Angeles County. The researchers matched six clinics into three groups, randomly allocating one clinic from each group to either intervention or control. The intervention clinics were randomly assigned one of the three reminder methods. The researchers determined the rates of vaccine uptake for the next dose and analyzed intervention outcomes using logistic regression models. The proportion of successfully delivered reminders for each method was used as a measure of feasibility. The study included 877 12-year-olds due for an HPV vaccine dose (47% female, more than 85% Latino). The study found that after a 4-month follow-up, 23% of those in the intervention group received an HPV vaccine dose, compared to just 12% in the control group. Overall, receiving any reminder led to higher rates of the next-required HPV vaccine compared to standard care (p = 0.046). Notable improvements were seen with SMS reminders (p = 0.036) and among boys (p = 0.006). Automated phone calls proved to be the least feasible method. Text message reminders are both effective and practical in encouraging HPV vaccination.

Antibody response to three SARS-CoV-2 mRNA vaccines in adolescent solid organ transplant recipients.
The purpose of this observational cohort study was to assess whether a third SARS-CoV-2 vaccine would improve immunogenicity in adolescent solid organ transplant recipients (SOTRs), and to report the antibody response and safety of a third mRNA vaccine dose (D3) in adolescent SOTRs. Forty-two participants received three BNT162b2 doses and one received three mRNA-1273 doses. Participants were 6-13 years from transplant (median 10 [IQR]), and heart transplant was the most common procedure, in 41.9% of the participants. 9.3% of participants reported pre-D1 SARS-CoV-2 infections and 9.3% reported breakthrough infections. The study found that 88.4% of adolescent SOTRs had positive antibody responses 1 month post-D3, an increase from 63–73% post-D2. 54.5% of participants with prior negative responses seroconverted and 100% with positive responses increased or remained at maximum titer. Titers remained stable 3 months post-D3. There were no vaccine-related adverse events and four breakthrough infections. The researchers concluded that although this was a convenience sample, the results suggest there is an antibody response benefit to a third SARS-CoV-2 vaccine in adolescent SOTRs.

Adolescents with vulvar ulcers: COVID-19 disease, COVID-19 vaccines, and the value of case reports.
The author indicates that there are too few cases reporting aphthosis after COVID disease or COVID-19 vaccination to infer a statistical association, but that case reports are a valuable source of rich details about conditions that are difficult to study with more rigorous designs and can be synthesized to help guide medical care. She recommends that it is time for a high-quality systematic review of vulvar aphthosis in order for clinicians to incorporate the existing evidence into decision-making and best care for patients.

Parent intentions to vaccinate children with autism spectrum disorder against COVID-19.
This study’s purpose was to investigate associations between parent vaccine confidence and intentions to have their child with autism vaccinated with the COVID-19 vaccine. A cross-sectional, web-based survey of 332 parents of children with autism spectrum disorder who were members of an integrated healthcare system in Southern California was conducted from May to July 2021. Approximately 35% of parents were going to vaccinate their child against COVID-19. Positive vaccine beliefs, healthcare provider trust, or parent vaccination status were associated with intention to vaccinate.

While many adult solid organ transplant recipients (SOTRs) have impaired antibody response to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination, pediatric SOTRs’ response has not been assessed. In this article, the researchers reported the immunogenicity and safety of BNT162b2 mRNA vaccination in pediatric SOTRs.

Increasing HPV vaccination among low-income, ethnic minority adolescents: effects of a multicomponent system intervention through a county health department hotline.
The human papillomavirus (HPV) vaccine has cancer prevention benefits, yet low uptake. The purpose of the study was to evaluate an intervention intended to improve vaccine uptake in low-income, ethnic minority adolescents using a telephone hotline to seek county health department services. The researchers recruited participants through randomization of health department hotline callers who were caregivers of never-vaccinated adolescents aged 11-17. The intervention included multi-lingual print and telephone education and personalized referral to a low cost or free provider of vaccines. Participants completed baseline, 3-month, and 9-month telephone surveys. The study found that by the end of the 9-month follow up period, the HPV vaccination rates had increased, however there were no differences between the intervention (45%) and control (42%) groups. The researchers also observed significant improvements in perceived HPV knowledge, perceived HPV risk, and barriers to vaccination. The study concluded that the county hotline intervention did not produce a greater increase in HPV vaccine rates in the intervention group than the group without the intervention. The study authors recommend that future studies should evaluate interventions which are more intensive and address accessing and using services in complex, safety net settings. The authors also noted that because 44% of unvaccinated adolescents in both the intervention and control groups received at least one dose of the vaccine during the study period, investigators of future studies should be aware of the potential priming effects of participation in the study, which may impact the results of interventions.

The risk of resurgence in vaccine-preventable infections due to coronavirus disease 2019-related gaps in immunization.
This paper discusses the decline in immunization delivery during the COVID-19 pandemic. The global community is at risk for a resurgence in vaccine-preventable infections including measles, pertussis, and polio. Outbreaks in the United States are likely to occur when social distancing stops and children return to school. Partnerships between healthcare providers, community leaders, and local, state, regional, and national public health departments are needed to reassure families that vaccine delivery during COVID-19 is safe as well as to identify and catch up those children who are under-immunized.

Associations between health care professional communication practices and influenza vaccination of hospitalized children.
The objective of this study was to describe the communication of health care professionals (HCPs) with parents of hospitalized children concerning the influenza vaccine. A survey was conducted online or via telephone with English- and Spanish-speaking parents of influenza vaccine-eligible children hospitalized at a pediatric hospital after discharge. Findings showed that a parent-reported HCP conversation and recommendation were associated with influenza vaccination during hospitalization.

Influenza-related knowledge, beliefs, and experiences among caregivers of hospitalized children.
Caregivers frequently decline influenza vaccine for their hospitalized child. In this study, the investigators aimed to examine factors impacting their influenza vaccine decision-making. The investigators conducted a cross-sectional survey study of English- and Spanish-speaking caregivers of children hospitalized at a tertiary care pediatric hospital between November 2017 and April 2018.They identified key factors impacting influenza vaccine decision-making among caregivers of hospitalized children, a critical step to improving uptake in this population.

Evaluation of a clinical decision support strategy to increase seasonal influenza vaccination among hospitalized children before inpatient discharge.
The authors’ goal was to design and evaluate a clinical decision support (CDS) strategy to increase the proportion of eligible hospitalized children who receive a seasonal influenza vaccine prior to inpatient discharge. They found that a user-centered CDS may be associated with significantly improved influenza vaccination rates among hospitalized children.

Childhood and adolescent vaccination in alternative settings.
This article reviews the unique advantages, challenges, and experiences regarding vaccine delivery in alternative settings, such as schools, emergency rooms, hospitals, and pharmacies. It describes the key components that each setting must possess as well as other important factors to consider when assessing the ability of each to deliver vaccines to the children and adolescents they serve. It is important to emphasize that these settings should not replace the medical home as the primary location for vaccination, but rather serve as a critical safety net for high-risk individuals and communities and in situations where access to traditional locations may be limited, such as during the COVID-19 pandemic.

A community divided: post-transplant live vaccine practices among Society of Pediatric Liver Transplantation (SPLIT) centers.
The goal of this study was to assess current post-transplant live vaccine practices at individual pediatric liver transplant centers following the updated AST guidelines. Using email surveys with a 93% response rate, findings showed that only 29% of centers offered live vaccines post-transplant, with each center using different eligibility criteria for live vaccines. The main reasons for a center not offering post-transplant live vaccines were safety concerns and inability to reach group consensus.

Under-immunization of pediatric transplant recipients: a call to action for the pediatric community.
This article is a call to action for pediatric solid organ transplant recipients to receive their recommended immunizations. Vaccine-preventable infections (VIPs) occur in 1 in 6 pediatric transplant recipients and these hospitalizations result in significant morbidity, mortality, graft injury, and cost. There has been an increase in the overall population in non-medical vaccine exemptions, which reduces herd immunity and creates greater risk for patients undergoing or already having received transplants.

Changes in otitis media episodes and pressure equalization tube insertions among young children following introduction of the 13-valent pneumococcal conjugate vaccine: a birth cohort-based study.
The impact of 13-valent pneumococcal conjugate vaccine (PCV13) introduction on the occurrence of first and subsequent otitis media (OM) episodes in early childhood is unclear. In this study, the investigators compared the risk of OM episodes among children age <2 years before and after PCV13 introduction, accounting for the dependence between OM episodes. They concluded that the transition from PCV7 to PCV13 was associated with a decline of OM among children aged <2 years due to a reduction in the risk of both the first and subsequent OM episodes.

Immunization status at the time of liver transplant in children and adolescents.
In this research letter, the authors quantified the percentage of patients at Society of Pediatric Liver Transplantation centers who were up to date for their age on immunizations at the time of transplant, and determined whether demographic and clinical factors were associated with immunization status at the time of transplant. They concluded that further research is needed to understand barriers to immunization in the pediatric transplant population and called for novel tools that provide pretransplant vaccine education and enhance communication between primary care and subspecialty clinicians.

Cost effectiveness of school-located influenza vaccination programs for elementary and secondary school children.
This paper describes a clinical trial which created a school-located influenza vaccination (SLIV) program in upstate New York. Researchers wanted to determine if this was a cost-effective alternative to vaccination in primary care practices. Two groups of students were vaccinated – one in an elementary school and the other in a secondary school. Secondary school vaccinations were more cost-effective than elementary schools with a lower median cost per vaccination. The authors concluded that it does raise vaccination rates, but it is not cost-effective due to the higher costs for consent systems and project coordination.

Text message reminders for child influenza vaccination in the setting of school-located influenza vaccination: a randomized clinical trial.
This study assessed whether text message reminders to parents could raise rates for children receiving school-located influenza vaccination (SLIV). Researchers randomized families with children in urban elementary schools to receive either text message plus SLIV (intervention) or SLIV alone (comparison). All parents were also sent two ‘backpack’ notifications and two autodialer phone reminders about SLIV at a single SLIV clinic. The text message reminders did not raise influenza vaccination rates above those observed with SLIV alone, and the authors conclude that more intensive interventions are needed to raise influenza vaccination rates.

Longer-term direct and indirect effects of infant rotavirus vaccination across all ages in the US; 2000 - 2013: analysis of a large hospital discharge dataset.
Rotavirus disease dramatically declined among children under 5 years of age since the rotavirus vaccine was introduced in 2006. In this study, data from the Healthcare Cost and Utilization Project State Inpatient Database were used to conduct a time-series analysis of monthly hospital discharges across age groups for acute gastroenteritis and rotavirus from 2000-2013, in order to elucidate population level impacts.

Stories about HPV vaccine in social media, traditional media, and conversations.
To understand how stories from media and social interactions shape parents' HPV vaccination decisions, the authors sought to characterize parents' exposure to these stories, as well as associations between story exposure and vaccination behavior. They found that stories of HPV vaccine harms may be associated more strongly with vaccination behavior than stories of HPV vaccine preventable diseases. They recommended that communication campaigns consider strategies to elevate stories of preventable diseases in social and traditional media.

State laws about pharmacists providing human papillomavirus (HPV) vaccines vary considerably, limiting many pharmacists' ability to provide this important cancer prevention service. In this study, the investigators characterized physician and parent support for pharmacist-provided HPV vaccination for adolescents who are past due for vaccination. The authors concluded that to increase support for HPV vaccination services, pharmacists should raise awareness about their immunization training and standardize vaccination protocols that ensure coordination with primary care.

Pharmacies versus doctors' offices for adolescent vaccination.
This study sought to understand the relative advantage of pharmacies compared to doctors' offices for delivering HPV vaccination to adolescents. The authors concluded that to be more appealing to parents as HPV vaccine providers, pharmacy providers within community and hospital settings should build on their relative advantage with respect to accessibility and enhance their appeal of their healthcare environment.

Methods for addressing "innocent bystanders" when evaluating safety of concomitant vaccines.
The need to develop methods for studying the safety of childhood immunization schedules has been recognized by the Institute of Medicine and Department of Health and Human Services. The recommended childhood immunization schedule includes multiple vaccines in a visit. A key concern is safety of concomitant (same day) versus separate day vaccination. This paper addressed a methodological challenge for observational studies using a self-controlled design to investigate the safety of concomitant vaccination.
