Expanding APHON's Pediatric Chemotherapy/Biotherapy Provider and Instructor program to Spanish-speaking countries: pilot series development and evaluation.
This article described the process of adapting and implementing Provider and Instructor program courses for standardized, comprehensive pediatric chemotherapy/biotherapy education in Latin American and Caribbean countries, culturally adapted and translated to Spanish by the Association of Pediatric Hematology/Oncology Nurses (APHON). The Spanish APHON Provider program was piloted four times and the Instructor program twice with nurses from Mexico, Central America, the Caribbean, South America, and Spain. Statistical analysis identified factors associated with pass rates. The Spanish APHON program was deemed appropriate for nurses' education and practice levels in Latin American and Caribbean countries.

A prospective observational study of video laryngoscopy-guided coaching in the pediatric intensive care unit.
The primary goal of this study was to evaluate whether implementation of video laryngoscopy-guided coaching for tracheal intubation in pediatric patients is feasible with a high level of compliance and associated with a reduction in adverse tracheal intubation-associated events. Findings showed that implementation of video laryngoscopy as a supervising device with standardized coaching language was feasible with a high level of adherence, yet was not associated with an increased occurrence of any adverse tracheal intubation-associated events and oxygen desaturation.

The current state of pediatric transplant hepatology fellowships: a survey of recent graduates.
This study’s goal was to describe the clinical, didactic, procedural, and research experiences of recent pediatric transplant hepatology (PTH) fellowship graduates. Findings showed that there is variability in the didactic, clinical, and procedural training among PTH fellowship programs. Although uniformly viewed as a beneficial fellowship year, there is an opportunity to collaborate to create a more standardized training experience.

Development of a quality improvement learning collaborative to improve pediatric sepsis outcomes.
Researchers developed a multicenter quality improvement learning collaborative of US children's hospitals. They created a key driver diagram (KDD) with the aim of reducing both the sepsis-attributable mortality and the incidence of hospital-onset sepsis in children. The KDD primary drivers focused on improving the following: treatment of infection; recognition, diagnosis, and treatment of sepsis; de-escalation of unnecessary care; engagement of patients and families; and methods to optimize performance. The Children's Hospital Association Improving Pediatric Sepsis Outcomes collaborative aims to improve sepsis outcomes through collaborative learning and reliable implementation of evidence-based interventions.

The new trainee effect in tracheal intubation procedural safety across PICUs in North America: a report from National Emergency Airway Registry for Children.
Researchers evaluated the effect of the timing of the PICU fellow academic cycle on tracheal intubation-associated events in a retrospective cohort study of 37 PICUs participating in the National Emergency Airway Registry for Children.. They found that the New Trainee Effect in tracheal intubation safety outcomes was not observed in various types of PICUs. There was a significant improvement in pediatric critical care medicine fellows' first attempt success and a significant decline in tracheal intubation-associated event rates, indicating substantial skills acquisition throughout pediatric critical care medicine fellowship.

Visiting Jack: mixed methods evaluation of a virtual home visit curriculum with a child with medical complexity.
This study’s objective was to evaluate the impact of participation in a virtual home visit curriculum on pediatric residents’ confidence, knowledge, and application of knowledge in caring for children with medical complexity (CMC) in the home and community. This prospective pre-post intervention study was conducted in 2019 with first-year pediatric residents using quantitative and qualitative methods. The intervention used was an online video-based curriculum followed by an in-person seminar. Twenty-four residents participated. All reported increased confidence in all aspects of complex care presented in the curriculum. Half of them (12) participated in a follow-up interview or focus group. Four themes identified were: 1) recognizing prior attitudes towards complexity; 2) new mental framework for mental care at home; 3) drivers of behavior change; and 4) commitment to change practice.

Research interest in pediatric emergency medicine fellows.
The authors sought to determine the prevalence and determinants of interest in research careers among pediatric emergency medicine (PEM) fellows. They performed an electronically distributed national survey of current PEM fellows, assessing demographics, barriers to successful research, and beliefs about research using 4-point ordinal scales. The authors found that most fellows expect to devote a minority of their career to clinical research. Enthusiasm about research was strongly correlated with career research interest.

Power of a learning network in congenital heart disease.
This paper discusses the efforts of the National Pediatric Cardiology Quality Improvement Collaborative (NPC-QIC). The collaborative formed to improve outcomes in infants with hypoplastic left heart syndrome. It sought to (1) decrease mortality, (2) reduce growth failure, and (3) reduce hospital readmissions due to major medical problems during the interstage period between discharge following stage 1 palliation (S1P) and admission for stage 2 palliation (S2P).

Characteristics associated with confidential consultation for adolescents in primary care.
This study examines how provider report of confidential consultation in the electronic health record is associated with adolescent characteristics, health risk factors, and provider training. The study found that provider training is needed to reinforce the importance of confidential consultation for all adolescents.

Leveraging medical conferences and webinars for hands-on clinical quality improvement: an intervention to improve health literacy-informed communication in pediatrics.
In this article, the authors describe the process and results with integrating a hands-on quality improvement learning session at the 2016 American Academy of Pediatrics meeting supplemented with virtual peer and expert support through webinars. Their results indicate that this process provided participants with the opportunity to improve clinical care through the use of health literacy–informed communication strategies.

The simulation-based assessment of pediatric rapid response teams.
The researchers created scenarios of simulated decompensating pediatric patients to train pediatric rapid response teams (RRTs) and to determine whether the scenario scores provide a valid assessment of RRT performance with the hypothesis that RRTs led by intensivists-in-training would be better prepared to manage the scenarios than teams led by nurse practitioners. The greater scores achieved by intensivist-in-training-led teams provides some evidence to support the validity of the assessment.

The development of tracheal intubation proficiency outside the operating suite during pediatric critical care medicine fellowship training: a retrospective cohort study using cumulative sum analysis.
The researchers hypothesized that both overall and first-attempt tracheal intubation success rates by pediatric critical care medicine fellows would improve over the course of training. They found that all fellows who completed 3 years of training during the study period achieved an acceptable 90% overall tracheal intubation success rate. They concluded that further investigations on a larger scale across different training programs are necessary to clarify intensity and duration of the training to achieve tracheal intubation procedural competency.

The unwritten rules of mentorship: facilitators of and barriers to effective mentorship in pediatric hospital medicine.
This study aimed to characterize successful pediatric hospitalists' past and current mentorship experiences and identify facilitators of and barriers to effective mentorship in pediatric hospital medicine (PHM). They found that several themes emerged regarding facilitators of and barriers to effective mentorship in PHM. These "unwritten rules of mentorship" may serve as a guide to establish and maintain beneficial mentorship relationships and overcome challenges.

Intern and resident workflow patterns on pediatric inpatient units: a multicenter time-motion study.
The researchers sought to quantify the proportion of time spent by residents in direct care, indirect care activities, and education across 9 pediatric institutions. They found that across all sites and levels of training, trainees spent more time in interprofessional communication (34.7 percent), and at the computer (20.5 percent), and less time in contact with patients and families (12.0 percent) and in educational activities (4.7 percent).

A virtual childhood obesity collaborative: satisfaction with online continuing education.
This descriptive study evaluated school-based health center (SBHC) providers’ satisfaction with Web based continuing education as part of a virtual childhood obesity intervention. Participation in the first two learning sessions was higher than the last two. Provider satisfaction of training modules by question type and content area was quite high. Many providers also reported plans to make changes in their practice after completing the training.

Residents' knowledge and comfort with caring for children with autism spectrum disorder.
The authors evaluated a cohort of pediatric residents’ knowledge of and comfort with caring for children with autism spectrum disorder (ASD). Their data suggest that more education is needed to increase resident competence in caring for individuals with ASD. Residents reported very low rates of direct communication with patients with ASD and no residents had used pictures to communicate with these patients. The authors found it concerning that few residents had found resources to help these patients with their communication needs. At the same time, comfort with using pictures to communicate was scored higher than other domains.

This editorial examines evaluation of the impact of pediatric supplemental crisis resource management (CRM) training among first- and second-year residents. The author suggests that increasing CRM training and using debriefing for skill retention may link this training to patient outcomes.
