Repeated depression screening during the first postpartum year.
The researchers report a substudy of a large pragmatic trial of early post-partum depression screening and practice management, the Translating Research into Practice for Postpartum Depression (TRIPPD) study. They concluded that repeated PPD screening at 6 and 12 months’ postpartum increases the percentage of women identified as being at high risk of PPD.

Rheumatoid arthritis is associated with higher ninety-day hospital readmission rates compared to osteoarthritis after hip or knee arthroplasty: a cohort study.
This study examined whether an underlying diagnosis of rheumatoid arthritis (RA) or osteoarthritis (OA) impacts the 90-day readmission rates after total hip arthroplasty (THA) or total knee arthroplasty (TKA). The 90-day post arthroplasty readmission risk after THA or TKA is higher in patients with rheumatoid arthritis (RA) compared to osteoarthritis.

Risks of herpes zoster in patients with rheumatoid arthritis according to biologic disease-modifying therapy.
This study evaluated whether the risks of herpes zoster (HZ) differed by biologic agents with different mechanisms of action (MOAs) in older rheumatoid arthritis (RA) patients. Using Medicare data from 2006–2011, the researchers found that among RA patients, the rate and adjusted hazard ratios of HZ were similar among biologic agents, including those with non–tumor necrosis factor mechanisms of action.

Safety and postoperative adverse events in pediatric otologic surgery: analysis of American College of Surgeons NSQIP-P 30-day outcomes.
This study described safety and postoperative sequelae of pediatric otologic surgery and identify predictive factors for postoperative events. It found that pediatric otologic procedures are common and have low rates of global 30-day postoperative events. Tympanomastoidectomy and cochlear implantation have the highest risk of 30-day readmission.

Seeking social support on Facebook for children with autism spectrum disorders (ASDs).
This study examined the types of social support messages exchanged between parents and/or caregivers of children with Autism Spectrum Disorders (ASDs) who communicate via Facebook. It found that the highest percentage of messages offered dealt with informational support (30.7 percent) followed by emotional support (27.8 percent). Network and esteem support messages were responsible for 20.97 percent and 20.2 percent, respectively.

This letter responds to a letter by Dr. Richard Young who criticizes Singh’s article on measuring diagnostic error. Singh defends his systems-based approach to advancing the science of measuring diagnostic error and acknowledges some of the uncertainties and evolution in the diagnostic process that Dr. Young writes about.

Short communication: Viral suppression is associated with increased likelihood of colorectal cancer screening among persons living with HIV/AIDS.
This retrospective longitudinal study evaluated patient characteristics associated with colorectal cancer (CRC) screening in an HIV cohort. During a median follow-up time of 1.7 years, only 30 percent of patients underwent CRC screening. The majority of screened patients received endoscopic screening (colonoscopy, 86 percent; sigmoidoscopy, 8 percent); results were available for 68 of the 75 patients, and adenomatous polyps were found in 13 percent. No cases of CRC were reported.

Signposts along the journey toward high-quality palliative care: the value of measuring what matters.
The author of this letter discusses the rationale behind and the usefulness of the group of palliative care measures that were chosen through the deliberations of a Technical Advisory Panel, a Clinical User Panel, and public comment. He states that the 10 measures chosen represent important signposts along the journey toward high quality palliative care and that this is the first iteration of this effort, with updates planned.

Socio-demographic and geographic variations in the utilization of hormone therapy in older women with breast cancer after Medicare Part-D coverage.
The researchers assessed socio-demographic, geographic, and other variations in the receipt of hormone therapy among patients with hormone receptor-positive breast cancer as well as adherence to hormone therapy within 1-year follow-up. They found that found that increasing age was significantly associated with a decreased receipt of hormone therapy and aromatase inhibitors in all hormone receptor-positive breast cancer patients after adjusting for other factors regardless of whether chemotherapy was received.

Sources of traffic and visitors' preferences regarding online public reports of quality: web analytics and online survey results.
Online public reports of quality exist, but little is known about how visitors find reports or about their purpose in visiting. To address this gap,the researchers gathered website analytics data from a national group of online public reports of hospital or physician quality and surveyed real-time visitors to those websites. They found that consumers were frequently interested in using the information to choose providers or assess the quality of their provider.

Stability of postoperative delirium psychomotor subtypes in individuals with hip fracture.
This paper's objectives were to determine the stability of psychomotor subtypes of delirium over time and to identify characteristics associated with delirium psychomotor subtypes in individuals undergoing surgical repair of hip fracture. The Transfusion Trigger Trial for Functional Outcomes in Cardiovascular Patients Undergoing Surgical Hip Fracture Repair Cognitive Ancillary Study was conducted. Results showed that psychomotor subtypes of delirium often fluctuated from assessment to assessment, rather than representing fixed categories of delirium. Hypoactive delirium was the most common presentation of delirium but the least likely to be documented by healthcare providers.

Surgeon case volume and 30-day mortality after carotid endarterectomy among contemporary medicare beneficiaries: before and after national coverage determination for carotid artery stenting.
The objective of this study is to examine the decline in past-year case-volumes of surgeons performing carotid endarterectomy (CEA) before and after the National Coverage Determination (NCD) for carotid artery stenting (CAS) and to assess its effect on 30-day post-CEA mortality. It found that the rate of CEA procedures decreased substantially during 2001 to 2008. The postprocedural mortality in Medicare beneficiaries was high compared with trial patients.

TELEmedicine for Patients with Inflammatory Bowel Disease (TELE-IBD): design and implementation of randomized clinical trial.
This study compares disease activity and quality of life over 1 year in a randomized trial of IBD patients receiving standard care versus telemedicine. If effective, telemedicine should decrease symptoms, improve quality of life, and decrease health care utilization. The burden associated with use of telemedicine for patients and providers needs to be assessed. This trial is ongoing and will be completed in July 2016.

The study objective was to identify how donors and government agencies in Vietnam responded to donor proliferation in health sector aid between 1995 and 2012. The study concludes that central government and donor agencies in Vietnam responded to donor proliferation in health sector aid by endorsing aid effectiveness policies but implementing these policies inconsistently in practice.

To test or not to test ... decision analysis of decision support.
In this article, the authors comment on a study by Troyer and colleagues in the same issue that presents a provocative decision analysis study exploring the cost and benefits associated with a web-based quantitative pretest probability calculator. Their discussion centers around the decision to test or not test in the emergency department setting for a condition. Such decisions are among the most consequential that physicians make and often hinge on advanced imaging.

Transforming patient-centered care: development of the evidence informed decision making through engagement model.
The purpose of this article is to set forth a new patient-centered implementation model informed by a qualitative study that explored women’s decisions, perceptions, and experiences of elective induction of labor. Through a process of theory evaluation and development, the authors explore patient-centered concepts (patient activation and shared decision making) within an implementation model by mapping qualitative data from an elective induction of labor study to assess the model’s ability to capture these key concepts.

Understanding why patients with COPD get readmitted: a large national study to delineate the Medicare population for the readmissions penalty expansion.
The Hospital Readmissions Reduction Program (HRRP) penalizes hospitals for 30-day readmissions and was extended to COPD in October 2014. The authors investigated readmission risk factors and reasons for readmission in order to guide hospitals in initiating programs to reduce COPD readmissions. They found that patients discharged home without home care were more likely to be readmitted for COPD than patients discharged to post-acute care, and those readmitted were more likely to be dually enrolled in Medicare and Medicaid, have a longer median length of stay, and have more comorbidities. They concluded that the addition of COPD to the readmissions penalty may further worsen the disproportionately high penalties seen in safety net hospitals.

Utility of socioeconomic status in predicting 30-day outcomes after heart failure hospitalization.
The researchers investigated whether accounting for socioeconomic status (SES) can improve risk-adjusted models for 30-day outcomes among CMS beneficiaries hospitalized with heart failure. They found that county-level SES data are modestly associated with 30-day outcomes but do not improve risk adjustment models based on patient characteristics alone.

Vitamin D sufficiency may be one of a myriad of host and environmental factors that can be directly impacted to reduce the frequency of S. aureus skin and soft tissue infection. The researchers found that children with vitamin D deficiency or insufficiency [25-hydroxyvitamin D <30 ng/mL] were more likely to present with recurrent, rather than primary, S. aureus skin or soft tissue infection.

When do patient-reported outcome measures inform readmission risk?
The study sought was to characterize changes in patient-reported outcome measures from hospital discharge to assess when they best inform risk of utilization as defined by readmissions or emergency department use. It concluded that routine measurement of patient-reported outcomes can help identify patients at higher risk for utilizations. For example, in-hospital assessments revealing high symptom burden and poor health status predicted 14-day reutilization.

The effect of an information and communication technology (ICT) on older adults' quality of life: study protocol for a randomized control trial.
The goal of this study is to test the effects of a technology called Elder Tree designed for and used by older adults and their family caregivers. The primary purpose of Elder Tree, a web-based information and communication technology (ICT), is to improve older adult quality of life. Recruitment for the trial began in November 2013 and the intervention period will end in November 2016.

Multiple perspectives on clinical decision support: a qualitative study of fifteen clinical and vendor organizations.
The purpose of this study was to discover how the views of clinical stakeholders, clinical decision support (CDS) content vendors, and EHR vendors are alike or different with respect to challenges in the development, management, and use of CDS. The groups share views on the importance of appropriate manpower, careful knowledge management, CDS that fits user workflow, and the need for communication among the groups.

Cystatin C and long term risk of community-acquired sepsis: a population-based cohort study.
The researchers sought to determine the association between elevated baseline Cyst-C and long-term rates of community-acquired sepsis. They found that elevated Cyst-C is associated with increased long-term rates of community-acquired sepsis, independent of abnormal eGFR, ACR or hsCRP. Cyst-C may play a role in long-term sepsis risk prediction and prevention.

The researchers explored the prospective effects of multimorbidity on health outcomes (health status, major health decline, and mortality). They found a strong and significant association between multimorbidity and prospective health status, major health decline, and mortality and concluded that multimorbidity may be used — both in clinical practice and in research — to identify older adults with heightened vulnerability for adverse outcomes.
