SOPS Ambulatory Surgery Center Database
AHRQ has established the Ambulatory Surgery Center Survey on Patient Safety Culture Database as a central repository for survey data from ambulatory surgery centers (ASCs) that have administered the AHRQ patient safety culture survey instrument and choose to submit their survey data to the AHRQ Ambulatory Surgery Center SOPS® database. The database serves as an important resource for patient safety culture improvement.
Participation is free and open to all SOPS ASC Survey users, provided the questionnaires are administered in a manner consistent with SOPS guidance and survey data are submitted according to SOPS specifications. The SOPS ASC Database contains data voluntarily submitted by participating ASCs and is not representative of all U.S. ASCs.

SOPS ASC Survey Database Report Current Results
2021 ASC Database Report (PDF, 4.2 MB); Executive Summary (PDF, 1 MB); Appendixes (PDF, 660 KB)—Presents data from 235 participating ASCs that administered the survey and submitted data. The report consists of a narrative description of the findings and two appendixes, presenting data by facility characteristics (number of operating/procedure rooms and geographic region) and respondent characteristics (organizational learning, staffing, work pressure and pace, and communication in the surgery/procedure room).
