2008 R21 Exploratory and Developmental Grants on Multiple Chronic Conditions

These grants were funded to begin to address the gap in knowledge about patients with multiple chronic conditions, with a focus on the use of preventive services. Additional funding in 2009 was awarded to two of the original 18 grantees to support collaborative activities. The grantees who received the additional funding follow this listing.
