Adolescent females' dyadic conversations about body, weight, and appearance.
The purpose of this observational study was to examine body talk between adolescent female dyads aged 13-17. Reciprocally nominated dyads were recruited from a southeastern US high school. Themes that emerged were related to weight, appearance, and personality. The authors concluded that their results provided insight into the social context in which sociocultural norms of weight stigma, body dissatisfaction, and eating-related psychopathology may be reinforced, and that these findings have implications for informing the development of interventions to reduce negative weight- and appearance-related body talk among adolescent girls.

The buffering effect of family support on the association between weight-based teasing and adolescent weight management outcomes.
This study examined the buffering effect that family support on the association between weight-based victimization (WBV) and adolescent weight management options. Parents of adolescents (n = 78) completed psychosocial measures at baseline of a clinical weight management program. Objective height and weight were measured at baseline and follow-up at Visit 3. There was a significant moderating effect of family support on the association between WBV and weight change (p = .04), accounting for 5.0% of the variance in weight change.

Formative development of a weight management intervention for adolescents with type 1 diabetes mellitus and obesity.
Researchers interviewed overweight or obese adolescents with type 1 diabetes mellitus (T1D), their caregivers, and pediatric endocrinologists to understand if they would be interested in a weight management intervention adapted for youth with T1D. The results provided detailed recommendations for the adaptation of a behavioral weight management intervention. Five central themes emerged: program content, programmatic messaging, program structure, social support, and risk of eating disorders.

Ultra-processed food consumption and BMI-Z among children at risk for obesity from low-income households.
The purpose of this study was to assess the relationship between baseline ultra-processed food consumption in early childhood and child BMI Z-score over 36 months. Among 595 children, median baseline age was 4.3 years, 65.4% had normal weight, 33.8% were overweight, and 0.8% were obese. When compared with low ultra-processed consumption (300 kcals/day), High ultra-processed intake (1300 kcals/day) was related with a 1.2 higher BMI-Z at 36 months for 3-year-olds and a 0.6 higher BMI-Z for 4-year-olds. The difference was not statistically significant for 5-year-olds or overall.

Engaging stakeholders to adapt an evidence-based family healthy weight program.
The purpose of this study was to describe the procedure of utilizing qualitative data from community and intervention stakeholders, children with overweight or obesity from low-income backgrounds, and caregivers to inform adaptations to the JOIN for ME pediatric weight management intervention. The researchers conducted qualitative interviews with 21 key community and intervention stakeholders, and conducted focus groups in both Spanish and English with 35 children with overweight or obesity from low-income backgrounds and 71 caregivers of children with overweight or obesity from low-income backgrounds. The results of the qualitative data analysis guided intervention modifications including content revisions to simplify and tailor materials, contextual revisions to improve intervention engagement and framing, resource awareness, and modality of delivery, training adaptations, and implementation/scale-up activities to improve community partner connections.

Weight management engagement for teens from low-income backgrounds: qualitative perspectives from adolescents and caregivers.
Adolescents from low-income backgrounds are at a higher risk for obesity and obesity-related negative health outcomes. In addition, these adolescents have lower access to, and success in, weight management (WM) programs. The purpose of this qualitative study was to explore engagement in a hospital-based WM program from the adolescent and caregiver perspective at varying levels of program initiation and engagement. The researchers conducted qualitative interviews with 55 participants, including 29 adolescents and 26 caregivers. This included: a) those who were referred to WM treatment, but never initiated (non-initiators); b) those who dropped out from treatment; and c) those who that had continuous participation in treatment (engaged). The study found that participants across all groups reported that they did not have a complete understanding of the scope or goals of the WM program after initial referral. In addition, many participants identified misperceptions of the program (e.g., perceptions of a screening visit as compared to an intensive program). Both caregivers and adolescents identified caregivers as drivers of engagement, with adolescents often tentative about participation in the program. However, engaged adolescents found the program valuable and sought ongoing participation following caregiver initiation.

Dynamo Kids!/¡Niños Dinámicos! A web site for pediatric primary care providers to offer parents of children 6-12 years old with overweight and obesity: web site development and protocol for pilot study.
The purpose of this study will be to determine whether a self-guided web site offered by pediatric primary care providers could assist parents with applying healthy behaviors. If successful, a large, controlled study may be conducted to further assess. The Dynamo Kids/¡Niños Dinámicos program was created with feedback from parents and health care professionals, developing a bilingual site for parents to about healthy behavior changes for their children and how to implement them. Electronic health record modifications alert providers to qualified children at well-child encounters. Parents complete online surveys before entering the site. In month 3 of the study, the practitioners will examine children in weight-focused appointments. Outcomes include change in Family Nutrition and Physical Activity parent survey score (primary) and change in child relative BMI (secondary). Additional data include time spent on site, provider surveys, and provider and parent interviews.

Multi-sector stakeholder's perceptions of determinants of successful implementation of a pediatric weight management intervention.
The objective of this study was to examine the perspectives of local, state, and national clinic and community stakeholders as an aid to developing successful pediatric weight management interventions (PWMI) in primary care and community settings, and to identify barriers and facilitators to implementation. Stakeholders in health centers and predominantly lower income, Hispanic community YMCAs were interviewed. Twenty-six of the stakeholders perceived formal curriculum with illustrative examples, patient- and family-centered programs, group visits, and high-quality multidisciplinary personnel to be components needed for a PWMI. These responses led to the creation of a group visit curriculum, implementation trainings, and cross-site collaborative technical assistance. The authors conclude that their findings highlight the importance of engaging multi-sector stakeholders during pre-implementation to ensure that valued components are included.

Enhancing quality measurement with clinical information: a use case of body mass index change among children taking second generation antipsychotics.
The authors sought to examine the extent to which body mass index (BMI) was available in electronic health records for Florida Medicaid recipients aged 5 to 18 years taking Second-Generation Antipsychotics (SGAP). They concluded that meeting the 2030 CMS goal of digital monitoring of quality of care will require continuing expansion of clinical encounter data capture to provide the data needed for digital quality monitoring. Using linked electronic health records and claims data allows identifying children at higher risk for SGAP-induced weight gain.

Association of changes in obesity prevalence with the COVID-19 pandemic in youth in Massachusetts.
Using the Massachusetts Department of Public Health’s disease surveillance system (MDPHnet), the authors examined obesity prevalence in 3 periods from 2018 to 2020 in a fixed cohort of children and adolescents. They found that, although childhood obesity prevalence was rising prior to COVID-19, the prevalence increased by a greater difference in Massachusetts youth during the COVID-19 pandemic. They observed greater increases in obesity prevalence in Black and Hispanic youth, particularly in boys aged 6 to 11 years.

Comparative effectiveness of clinical and community-based approaches to healthy weight.
The objective of this study was to evaluate if two pediatric weight management interventions delivered to Hispanic, low-income children (one in a health center, the other in a Young Men's Christian Association) were effective in reducing BMI. The authors hypothesized that both would be equally effective. They were unable to establish noninferiority of the modified Healthy Weight and Your Child program. The healthy weight clinic at the health center improved BMI, offering an effective treatment of those disproportionately affected.

Adolescent weight management intervention in a nonclinical setting: changes in eating-related cognitions and depressive symptoms.
The present study was a secondary data analysis of a randomized controlled trial (RCT) to examine changes in depressive symptoms and eating-related cognitions in teens who participated in a non-clinic-based adolescent behavioral weight control treatment delivered by YMCA coaches. The results demonstrated some positive and no detrimental effects of a non-clinic-based behavioral weight control intervention on adolescents' eating-related cognitions and depressive symptoms.

Characteristics of achieving clinically important weight loss in two paediatric weight management interventions.
The purpose of this study was to examine characteristics and lifestyle behaviors associated with achieving clinically important weight loss (CIWL) in two pediatric weight management interventions (PWMIs). Findings showed that, in this analysis of children enrolled in PWMIs, achieving CIWL was associated with younger age, not having severe obesity, and consuming fewer sugary beverages at the end of the intervention.

Pilot study of a brief provider and EMR-based intervention for overweight teens with asthma.
The authors piloted an electronic medical record-based tailored discussion guide (TDG) and a brief provider training to address weight management in overweight teens with asthma. They observed modest improvements in patient-reported asthma outcomes and health behaviors. They found strong evidence that the TDG supports provider discussion of weight and asthma to create a more patient-centered conversation from the perspective of participating teens. They recommended addressing challenges to recruitment and clinic adaptation prior to advancing to a full-scale trial.

Community-based sugar-sweetened beverage intervention associated with short-term improvements in self-rated oral health.
The purpose of this study was to assess the efficacy of a community-based childhood obesity prevention intervention targeting sugar-sweetened beverage (SSB) consumption on self-rated oral health among children and their parents/caregivers. The investigators found that short-term improvements in oral health among children and their parents/caregivers were observed among those participating in a SSB behavioural intervention.

Reactions to the U.S. Preventive Services Task Force guidelines for the management of adolescent obesity: a multi-informant qualitative approach.
This study measured reactions by adolescents, parents, and physicians to the U.S. Preventive Services Task Force guidelines for the management of adolescent obesity. The guidelines recommended those identified should be referred to comprehensive weight management programs consisting of at least 26 contact hours and focus on multiple targets of behavior. In-depth interviews were conducted with 7 adolescents, 7 parents, and 4 physicians. In general, participants viewed the guidelines positively but identified changes that may be necessary to increase the feasibility of adolescents engaging in programs that meet these guidelines. Participants also noted the importance of flexibility within programs.

Attention deficit hyperactivity disorder medications and BMI trajectories: the role of medication type, sex and age.
Investigators estimated the proximal change in BMI associated with start of medication and to assess whether medication-specific departures in BMI varied by age and sex in patients with attention deficit hyperactivity disorder (ADHD). Longitudinal electronic health records from children aged 4-19 years with an ADHD diagnosis at one healthcare system were analyzed. They found that all medications had larger impacts at younger ages; decreased BMI was observed with stimulants, while antidepressants and antipsychotics led to BMI increases as did alpha-agonists, significantly varying by sex and age. They recommended future research to study further the interactions of these medications on long-term impacts on obesity.

Executive function phenotypes in pediatric obesity.
This study examined the behavioral phenotypes of children with and without executive function (EF) impairments in a clinical sample of youth aged 8-17 years attending a medical clinic for obesity. A caregiver-proxy report of EF was assessed using the Behavior Rating Inventory of Executive Function. Four latent classes of EF impairment were identified: No/Low Impairment, Behavioral Regulation Impairment, Metacognition Impairment, and Global Impairment. Behavioral/emotional impairments tended to increase with EF impairment. Children with obesity and EF impairment were found to have a dysregulated behavioral phenotype ranging from internalizing to externalizing behavioral and weight-related symptoms.

Negative childhood experiences and disordered eating in adolescents in a weight management program: the role of depressive symptoms.
This cross-sectional study examined the association between stressful life events and weight-related outcomes including weight status, disordered eating behaviors, and insulin sensitivity in treatment-seeking adolescents with overweight and obesity. The authors were also looking at the potential mediating role of depression. The study included 170 adolescents (mean age 14.8, 62% female) enrolled in an interdisciplinary weight management program. Findings were that stressful childhood experiences were significantly related to weight status and disordered eating but not insulin sensitivity. Depressive symptoms were related to stressful experiences and disordered eating patterns but not weight status or insulin sensitivity.

Unmet social needs and adherence to pediatric weight management interventions: Massachusetts, 2017-2019.
This study examined the effects of housing insecurity and unmet social needs on adherence to pediatric weight management intervention (PWMI) programs. The authors used data from children enrolled in a 2017-2019 comparative effectiveness trial for 2 high-intensive PWMIs in Massachusetts. Families with housing security had higher contact hours with the program than families without. Children with 3 to 4 unmet social needs (parental stress, parental depression, food insecurity, and housing insecurity) also attended less hours of the program than those without.

Using simulation modeling to guide the design of the Girl Scouts Fierce & Fit Program.
The goal of this study was to see if computational modeling would help to revise the design of the Girl Scouts Fierce & Fit Program. The Girl Scouts of Central Maryland worked with the researchers to develop a computational simulation model representing the 250 adolescent girls who were participating in the program to determine how their diets and physical activity affected their BMI and subsequent outcomes, including costs. As a result, the program changed from 6 weeks meeting twice a week with 5 minutes of physical activity each session to meeting 12 weeks with 30 minutes of physical activity each session. Cost savings were estimated at an additional $84,828 in lifetime direct medical costs, $81,365 in lifetime productivity losses, and 7.85 quality-adjusted life-years.

Family-centeredness of childhood obesity interventions: psychometrics & outcomes of the family-centered care assessment tool.
The purpose of this study was to evaluate the psychometric properties of a modified version of the Family Centered-Care Assessment (mFCCA) tool and to assess the family-centeredness of two clinical-community childhood obesity interventions. Using the mFCCA which demonstrated good psychometric properties for the assessment of family-centered care among parents of children with obesity, the investigators found that individualized health coaching is a family-centered approach to pediatric weight management.

Differences in frequency of visits to pediatric primary care practices and emergency departments by body mass index.
The purpose of this retrospective study was to examine differences in utilization across health care settings among children by body mass index (BMI) categories to help identify opportunities for interventions. The authors concluded that the children's BMI categories were associated with health care utilization, specifically primary care visits, ED visits, and hospitalizations.

Annual report on children's healthcare: healthcare access and utilization by obesity status in the United States.
This study examined access to care and utilization patterns across a set of healthcare measures by obesity status and sociodemographic characteristics among children. Data from the Medical Expenditure Panel Survey (MEPS) from 2010-2015 was used to determine obesity status, number of well-child visits, access to a usual source of care, number of preventive dental visits and prescription medication fills in the past year. It was found that uninsured adolescents with obesity were less like to have a usual source of care provider than children without obesity. Among younger children, those living in the Northeast were more than twice as likely to have had a well-child visit than those living in the West. Preventive dental care was less likely to have occurred for children with obesity than non-obese children. More prescription refills were completed for obese adolescents than for younger children.

Association of clinician behaviors and weight change in school-aged children.
This study used clinical practice data to determine whether recommended weight management clinician behaviors were associated with weight status improvement in children aged 6-12 years who are overweight or obese. Electronic health record data (2009-2014) from 52 clinics were used. Weight status was examined from 1 visit to the next as dichotomous improvement (versus worsening or no change) and change in percentage overweight (over sex/age-specific BMI95).
