SOPS Medical Office Database
AHRQ established the Surveys on Patient Safety Culture® (SOPS®) Medical Office Survey Database as a central repository for survey data from medical offices that have administered the AHRQ patient safety culture survey instrument and choose to submit their survey data to the AHRQ SOPS Medical Office Survey Database. The database serves as an important resource for patient safety culture improvement.
Participation in the SOPS Databases is free and open to users of the surveys to voluntarily submit their data to the appropriate database. In addition, organizations that administer any of the supplemental items with the companion SOPS survey can now also submit data to the appropriate database. Participating organizations receive a customized feedback report of their own results compared with the Database results. The SOPS Medical Office Survey Database contains data voluntarily submitted by participating medical offices and is not representative of all U.S. medical offices.

SOPS Medical Office Current Database Report Results
2026 Medical Office Survey Database Report (PDF, 3 MB); Highlights (PDF, 1 MB); Appendixes (PDF, 2 MB)—Presents data from 2,081 U.S. medical offices that administered the survey and submitted data for 25,040 providers and staff. The report consists of a narrative description that details the findings and two appendixes, presenting data by medical office characteristics, and respondent characteristics.

2024 Results for the AHRQ SOPS Diagnostic Safety Supplemental Item Set for Medical Offices (PDF, 1.6 MB); Executive Summary (PDF, 1.2 MB)—Presents results for the Medical Office Diagnostic Safety Supplemental Item Set, including data submitted to the 2024 Medical Office Database for a total of over 700 providers and staff in 102 medical offices.
Results From the 2014 Pilot Study of the AHRQ SOPS Value and Efficiency Supplemental Item Set for the Medical Office Survey (PDF, 1 MB)—Presents data from 96 medical offices that administered and submitted data for more than 1,400 medical office staff in 2014. The report consists of a narrative description of the findings and an appendix explaining calculations.
