Nursing Home Survey on Patient Safety Culture

In response to nursing homes interested in a survey that focuses on patient safety culture in their facilities, AHRQ sponsored the development of the Nursing Home Survey on Patient Safety Culture. This new survey is designed specifically for nursing home providers and staff and asks for their opinions about the culture of patient safety in their nursing home. It is not designed for use in assisted living facilities, community care facilities, or independent living facilities.

Survey User's Guide

The User's Guide provides a general overview of the issues and major decisions involved in conducting a survey and reporting the results. The User's Guide includes information on getting started, selecting a sample, determining data collection methods, establishing data collection procedures, conducting a Web-based survey, and preparing and analyzing data, and producing reports.

Comparative Database

Many nursing homes using the AHRQ Nursing Home Survey on Patient Safety Culture have expressed interest in comparing their results to other nursing homes. In response, the Agency for Healthcare Research and Quality has established the Nursing Home Survey on Patient Safety Culture Comparative Database. This database is a central repository for survey data from nursing homes that have administered the AHRQ patient safety culture survey instrument.

Nursing Home Survey on Patient Safety Culture: 2011 User Comparative Database Report ( Part 1 [ PDF file - 1.19 MB]   Part 2 [ PDF file - 633 KB] —The first edition of the AHRQ Nursing Home Survey on Patient Safety Culture User Comparative Database Report presents data from 16,155 staff within 226 U.S. nursing homes. The 2011 report provides results that nursing homes can use to compare their patient safety culture to other U.S. nursing homes. The full report contains detailed comparative data for various nursing home characteristics (size and ownership) and respondent characteristics (job titles, work areas, direct patient contact, and shift worked).

2008 Preliminary Comparative Results: Nursing Home Survey on Patient Safety Culture ( PDF file PDF version - 419.16 KB )—Preliminary comparative results are provided for the items and safety culture dimensions on the AHRQ Nursing Home Survey on Patient Safety Culture to allow nursing homes to compare their survey results against the results from 40 nursing homes that participated in a pilot test of the survey in the United States in late 2007 and early 2008.

Technical Assistance Conference Call

A national technical assistance conference call conducted on March 31, 2010, provides users with an overview of the development of the survey. In addition, two professionals share their success and challenges in survey administration from both a regional perspective and an individual nursing home, and discuss how to maximize survey response and use survey results to develop action plans to target safety culture improvement initiatives.
