Goals/Purpose:
The central purpose of this project was to establish a national, web-based, pathologist-driven database to identify, analyze, and reduce anatomic pathology diagnostic errors and improve patient outcomes through quality improvement and error reduction initiatives.
Specific Aims:
- Identify archival and ongoing diagnostic errors and their associated outcomes retrospectively at four institutions.
- Generate and disseminate quarterly and annual quality performance reports based on the error and outcome data.
- Conduct root cause analysis to determine the underlying sources of diagnostic errors.
- Evaluate the effectiveness of the interventions by monitoring post-intervention diagnostic error rates and assessing the impact of these errors on patient outcomes.
