A key component of external support to primary care practices to improve quality of care
EvidenceNOW Projects

Managing Unhealthy Alcohol Use
Screening, brief interventions, and treatment for unhealthy alcohol use in primary care
The EvidenceNOW Model of external support to primary care practices uses practice facilitation, expert consultation, and other services to help primary care practices implement evidence into practice.
Practice Facilitation is the use of practice facilitators, who are external consultants to primary care practices to help them implement evidence into practice and improve their capacity for quality improvement.
EvidenceNOW projects all use the model of external support to primary care practices to implement evidence into practice. The projects range from advancing heart health to managing unhealthy alcohol use and onsurgical treatment of urinary incontinence.
