Feasibility of implementing group well baby/well woman dyad care at federally qualified health centers.
The aim of this qualitative study was to assess stakeholder perspectives on the feasibility of implementing Centering Parenting (CP) in federally qualified health centers (FQHCs) in Baltimore. The investigators concluded that perceptions regarding facilitators and barriers to CP implementation in FQHCs were similar to existing group well-child care literature; however the benefit of emphasis on maternal wellness was a unique finding. The investigators suggested that a maternal wellness integration might make CP a particularly desirable model for implementation at FQHCs, but potential systems barriers must be addressed.

Emergency department visits for postpartum complications.
The investigators performed a retrospective analysis of all postpartum visits to the emergency department (ED) at a tertiary care women's hospital over 6 months and described characteristics of the population and clinical details of the ED visit, specifically the presenting complaint, delivery type, final diagnosis, and admission rate. They found that women presenting to the ED postpartum period had a wide variety of medical issues but 78% were not admitted.

Clinical management of women with monogenic diabetes during pregnancy.
This study discusses clinical management of women with monogenic diabetes during pregnancy. Monogenic diabetes is rare and only accounts for 1-2% of all diabetes cases so it is frequently misdiagnoses as one of the other diabetes types. Diabetes treatment is different for this type of diabetes, and if untreated can cause fetal mutations. However, if treated there can be transplacental transfer of the medication (sulfonylurea). The study authors recommend large prospective studies be conducted to better define the need and timing of initiation of insulin treatment.

Paid maternity leave in the United States: associations with maternal and infant health.
Using data from Listening to Mothers III, a national survey of women ages 18-45 who gave birth in 2011-2012, the investigators conducted multivariate logistic regression to predict the likelihood of outcomes related to infant health, maternal physical and mental health, and maternal health behaviors by the use and duration of paid maternity leave. They concluded that Practice Paid maternity leave significantly predicted lower odds of maternal and infant re-hospitalization and higher odds of doing well with exercise and stress management.

Differentiating research, quality improvement, and case studies to ethically incorporate pregnant women.
This article discusses the need to involve pregnant women in research, quality improvement, and case studies and how to involve them in an ethical way with high standards to protect participants. This review includes vignettes to distinguish between the different types of studies and emphasizes that perinatal care providers will need to seek institutional review board approval for all research to be conducted.

Effectiveness of a pediatric primary care intervention to increase maternal folate use: results from a cluster randomized controlled trial.
This study assessed the impact of provision of folate vitamins and a preconception health intervention on folate use among a group of mostly African-American and low-income mothers bringing infants to pediatric primary care. It found that among all participants, daily vitamin intake increased from baseline to 6-month follow-up (33.8 percent vs 42.6 percent).

Pregnancy-associated stroke.
This article reviews assessment and treatment of pregnant and postpartum women experiencing stroke. There are 2 main types of stroke, ischemic and hemorrhagic that present similar symptoms but have very different pathophysiology and treatment. This article provides guidance for subsequent maternity and primary care for front-line perinatal care providers.

Emergency department visits for postpartum hypertension.
The purpose of this study was to describe the characteristics of women diagnosed with postpartum hypertension in an emergency department (ED) to better inform postpartum care. The investigators compared women with an ED diagnosis of hypertension to women with all other ED diagnoses and concluded that postpartum surveillance may not prevent readmission for hypertension.

Health behaviors among women using fertility treatment.
The researchers described associated perinatal behaviors among women using fertility treatment. They concluded that a significant proportion of women using fertility treatments were not practicing recommended perinatal behaviors or receiving prenatal counseling on preventable risk factors.

Optimizing preconception health among young women: what are we missing?
This study evaluated whether receipt of specific preconception counseling topics differs between teen, young adult, and older mothers. It found that teen mothers were less than half as likely to be counseled about taking folic acid, 4 times more likely to be counseled about vaccines, and twice as likely to be counseled about mental health before pregnancy compared with adult mothers.

Using social media, online social networks, and internet search as platforms for public health interventions: a pilot study.
The researchers analyzed public health interventions aimed at women potentially interested in maternity care via campaigns on social media (Twitter), social networks (Facebook), and online search engines (Google Search). They found that the campaigns reached a little more than 140,000 consumers each day across the three platforms, with a little more than 400 engagements each day. Facebook and Google search had broader reach, better engagement rates, and lower costs than Twitter.

Examining the link between women's exposure to stressful life events prior to conception and infant and toddler health: the role of birth weight.
The authors suggested a pathway may exist among maternal exposure to stressful life events prior to conception (PSLEs), infant birth weight, and subsequent offspring health. Infant and toddler health outcomes were assessed at 9 and 24 months, including overall health status, special healthcare needs, and severe health conditions. The analysis suggested a chains-of-risk model in which women's exposure to PSLEs increased the risk for giving birth to a very low birth weight infant, adversely affecting infant and toddler health.

Screening for iron deficiency anemia and iron supplementation in pregnant women to improve maternal health and birth outcomes.
This case study involves a 28-year-old woman presents for a routine antepartum visit. She is 33 weeks pregnant and also has a four-year-old son. She has no particular problems, and the findings from your examination are unremarkable. She asks if her blood iron level should be tested. Based on the recommendations of the U.S. Preventive Services Task Force, the quiz asks 3 questions involving screening and treatment.

Association of adverse pregnancy outcomes with glyburide vs insulin in women with gestational diabetes.
The purpose of this study was to estimate the risks of adverse pregnancy outcomes among women receiving glyburide compared with insulin for the treatment of gestational diabetes mellitus (GDM) in a US population-based cohort. It found an association between glyburide (compared with insulin) and elevated risk of NICU admission, neonatal hypoglycemia, respiratory distress, birth injury, and large for gestational age in women with GDM.

Chronic hypertension in pregnancy and the risk of congenital malformations: a cohort study.
The researchers sought to examine the effect of chronic hypertension on the risk of congenital malformations in a large cohort of pregnancies in Medicaid beneficiaries. In a cohort of 878,126 pregnancies, they found that both treated and untreated maternal chronic hypertension were associated with a similar 20-30 percent increase in the risk of congenital malformations in the infant after adjusting for confounding factors as compared with normotensive controls.

Breastfeeding in African Americans may not depend on sleep arrangement: a mixed-methods study.
The researchers described the association between breastfeeding and bedsharing, elucidating barriers to breastfeeding in African Americans. They found that lower socioeconomic status mothers were more likely to breastfeed exclusively or at all if they bedshared; however, bedsharing was not associated with breastfeeding among higher socioeconomic status mothers. Breast pain, lack of support, and maternal skepticism about breastfeeding benefits were barriers.

Preconception health promotion among Maryland women.
The objective of this study was to identify factors associated with receipt of preconception care (PCC) health promotion counseling among Maryland women and to assess whether prior birth outcome affects receipt of counseling. PCC receipt in the sample, consistent with analyses of older data, was low at 33 percent for all women and 28 percent for women with a prior live birth.

Trends in glyburide compared with insulin use for gestational diabetes treatment in the United States, 2000-2011.
The researchers sought to characterize pharmacologic treatment of women with gestational diabetes (GDM) by describing trends in the use of glyburide compared with insulin over the past decade (2000-2011) and identifying predictors of initial choice of pharmacotherapy. They found that glyburide has replaced insulin as the more common pharmacotherapy for GDM among those privately insured.
