Community social determinants and health outcomes drive availability of patient-centered medical homes.
This study assessed the geographic distribution of patient-centered medical homes and the community social determinants and health outcomes that drive availability. The authors assessed spatial clusters of mental and physical health surveys; health behaviors; as well as premature mortality with clusters of medical home saturation and community socioeconomic characteristics. Counties having lower uninsured rates and lower poverty rates were more likely to have medical homes.

Factors influencing women's perceptions of shared decision making during labor and delivery: results from a large-scale cohort study of first childbirth.
The researchers examined correlates of shared decision making during labor and delivery. They found that women who were black and who did not have a college degree or private insurance were less likely to report high shared decision making, as well as women who underwent labor induction, instrumental vaginal or cesarean delivery.

Incorporating patients' social determinants of health into hypertension and depression care: a pilot randomized controlled trial.
The objective of this study was to carry out a randomized controlled pilot trial to test the effectiveness of an integrated intervention for hypertension and depression incorporating patients' social determinants of health (enhanced intervention) versus an integrated intervention alone (basic intervention). The pilot trial results indicate integrated care management that addresses the social determinants of health for patients with hypertension and depression may be effective.

Integrating social determinants of health into primary care clinical and informational workflow during care transitions.
Care continuity during transitions between the hospital and home requires reliable communication between providers and settings and an understanding of social determinants that influence recovery. This paper describes the coordinating transitions intervention which uses real time alerts, delivered directly to the primary care practice for complex chronically ill patients discharged from an acute care setting, to facilitate nurse care coordinator led telephone outreach.

Housing, transportation, and food: how ACOs seek to improve population health by addressing nonmedical needs of patients.
The authors examined how accountable care organizations (ACOs) addressed the nonmedical needs of their patients. They found that ACOs most commonly addressed the need for transportation, housing, and food insecurity, which they identified through the primary care visit or care transformation programs. They concluded that their findings offer insights into how health care organizations such as ACOs integrate themselves with nonmedical organizations.
