The year in review: the best of 2016 in the Annals.
Last year brought a large increase in publications on the topics of rhinitis and rhinosinusitis. However, new therapeutics are always of great interest to the practicing clinician and thus top the list. Also discussed are significant articles on the topics of asthma and lower airway diseases, systemic allergic and immune disorders, and allergen immunotherapy.

Translating behavior change techniques to new delivery mediums.
The translation of successful behavior change interventions to new delivery mechanisms requires an understanding of the underlying concepts that led to its success. A communication module within a multi-user telehealth kiosk is used as an example of how behavior change techniques from a "live" intervention may be represented in an information technology-delivered intervention.

Trends in racial-ethnic disparities in access to mental health care, 2004-2012.
This study compared trends in racial-ethnic disparities in mental health care access among whites, blacks, Hispanics, and Asians by using the Institute of Medicine definition of disparities as all differences except those due to clinical appropriateness, clinical need, and patient preferences. The investigators found that significant disparities were found in 2004-2005 and in 2011-2012 for all three racial-ethnic minority groups compared with whites in all three measures of access.

Trimethylamine N-oxide and cardiovascular events in hemodialysis patients.
This study determined the association of the proatherogenic metabolite trimethylamine N-oxide (TMAO) with cardiovascular outcomes in hemodialysis patients and assessed whether this association differs by race. It found that, compared with blacks in the same quintile, whites in the highest quintile for TMAO (>/=135 muM) had a 4-fold higher risk of cardiac or sudden cardiac death and a 2-fold higher risk of any-cause death.

Understanding treatment gaps for mental health, alcohol, and drug use in South Dakota: a qualitative study of rural perspectives.
This study explored how rural participants with behavioral health conditions pursue and receive care, and it examined how these factors differed across American Indian (AI) and geographic subpopulations. Seeking mental health care or maintaining sobriety was viewed as the result of an individual's willpower and frequently related to a substantial life event (eg, childbirth). Participants recommended treatment gaps be addressed through multicomponent community-level interventions.

Use of daily interruption of sedation and early mobility in US hospitals.
Although the Awakening and Breathing Coordination, Delirium assessment, and Early exercise/mobility (ABCDE) bundle may be effective, individual components of ABCDE may not be implemented as intended. The researchers examined the use of daily interruption of sedation and early mobility, looking for an association between these bundle elements.

Visit-to-visit systolic blood pressure variability and microvascular complications among patients with diabetes.
This study examined the relationship between systolic blood pressure (SBP) variability and the risk of microvascular complications in a non-elderly diabetic population. Compared to individuals with the least SBP variability (Quartile 1), those with most variability (Quartile 4) had 81 percent and 19 percent higher incidence of nephropathy, retinopathy, neuropathy, and any complication, respectively, after adjusting for mean SBP, demographic and clinical factors.

Vital signs are still vital: instability on discharge and the risk of post-discharge adverse outcomes.
This study assessed the association between vital sign instability at hospital discharge and post-discharge adverse outcomes. Having two or more vital sign instabilities at discharge had a positive predictive value of 22 percent and positive likelihood ratio of 1.8 for 30-day death or readmission. Vital sign instability on discharge is thus associated with increased risk-adjusted rates of 30-day mortality and readmission.

What factors influence states' capacity to report children's health care quality measures? A multiple-case study.
The objective of this study was to describe factors that influence the ability of state Medicaid agencies to report the Centers for Medicare & Medicaid Services' (CMS) core set of children's health care quality measures . Reporting capacity was influenced by a state's Medicaid data availability, ability to link to other state data systems, past experience with quality measurement, staff time and technical expertise, and demand for the measures.

Whole-exome sequencing to identify novel biological pathways associated with infertility after pelvic inflammatory disease.
The researchers used whole-exome sequencing to identify biological pathways that may be associated with STI-related infertility. Pathway analysis revealed that focus genes were enriched in canonical pathways including, IL-1 signaling, P2Y purinergic receptor signaling, and bone morphogenic protein signaling. Targeted sequencing of biological pathways identified in this study may provide insight into STI-related infertility.
