Minnesota's early experience with medical home implementation: viewpoints from the front lines.
This study aimed to identify the facilitators and barriers encountered by nine diverse primary care practices selected from the first 80 to achieve patient-centered medical home (PCMH) certification in Minnesota. It found that facilitators to achieve PCMH certification included a requirement for patient involvement, which pushed practices to create patient-centered innovations, and new reimbursement models based on quality indicators for a population.

Sustainability of evidence-based healthcare: research agenda, methodological advances, and infrastructure support.
This project identifies the challenges associated with sustainability research and generates recommendations for accelerating and strengthening this work. Its recommendations fell into three domains: (1) pursue high priority research questions as a unified agenda on sustainability; (2) advance methods for sustainability research; (3) advance infrastructure to support sustainability research.

Bringing patient-centered outcomes research to life.
Achieving better health outcomes requires both the generation of new patient-centered outcomes research (PCOR) knowledge and the appropriate and timely dissemination of that knowledge into practice. This article highlights the work AHRQ has done, is doing, and will do to improve the uptake of PCOR findings.

Disseminating policy and environmental change interventions: insights from obesity prevention and tobacco control.
This paper describes the approach that two projects developed to disseminate policy and environmental change interventions. The Center for Training and Research Translation (Center TRT) disseminates evidence-based interventions (EBIs) to promote physical activity and healthy eating. Counter Tobacco disseminates EBIs to counter tobacco product sales and marketing in the retail environment.

Translating an evidence-based behavioral intervention for women living with HIV into clinical practice: the SMART/EST Women's Program.
The researchers explored the pathways to effectively transfer promising research accomplishments into effective and sustainable service programs within the health care delivery system. Their study confirmed (a) the translatability of the Stress Management And Relaxation Training/Emotional Supportive Therapy (SMART/EST) Women's Program, from academic to community health center settings in two geographic regions with high HIV prevalence among women, (b) the ability of local staff to successfully achieve program fidelity and clinical outcomes, and (c) the sustainability the program beyond the auspices of research support, through supportive community health center leadership securing continued program funding.

Repeated depression screening during the first postpartum year.
The researchers report a substudy of a large pragmatic trial of early post-partum depression screening and practice management, the Translating Research into Practice for Postpartum Depression (TRIPPD) study. They concluded that repeated PPD screening at 6 and 12 months’ postpartum increases the percentage of women identified as being at high risk of PPD.

From strategy to action: how top managers' support increases middle managers' commitment to innovation implementation in health care organizations.
The researchers surveyed and interviewed middle managers implementing an innovation intended to reduce health disparities in 120 U.S. health centers to assess whether top managers’ support directly influences middle managers’ commitment. Their results suggest that top managers increase middle managers’ commitment by directly conveying to middle managers that innovation implementation is an organizational priority, as well as by several other means.

Moving the needle forward: the imperative for collaboration in vascular access.
The authors discussed vascular access patterns of use, complications, and outcomes. They recognized a need to create a better intersection between research evidence and clinical practice. They concluded that bedside and vascular access nurses, nurse researchers, and physician-scientists must partner in a way that generates shared questions, promotes transparency of the research process, and generates mutually beneficial products.
