Disseminating policy and environmental change interventions: insights from obesity prevention and tobacco control.
This paper describes the approach that two projects developed to disseminate policy and environmental change interventions. The Center for Training and Research Translation (Center TRT) disseminates evidence-based interventions (EBIs) to promote physical activity and healthy eating. Counter Tobacco disseminates EBIs to counter tobacco product sales and marketing in the retail environment.

Prepregnancy obesity and breastfeeding noninitiation in the United States: an examination of racial and ethnic differences.
This study used a large, nationally representative sample to assess racial/ethnic differences in breastfeeding noninitiation among U.S. women of different body mass index (BMI) classifications. It found that prepregnancy BMI is a significant predictor for breastfeeding noninitiation for non-Hispanic white and non-Hispanic black women.

A randomized, home-based, childhood obesity intervention delivered by patient navigators.
This paper delineates the study protocol for the Community Outreach Obesity Prevention Trial. COOPT is an ongoing, 4-year (October 2011-September 2015) randomized controlled trial that tests the effectiveness of a home-based patient navigator program delivered to preschoolers of a large urban safety-net health care system. The researchers believe that its home-based intervention venue will provide rich data characterizing barriers and facilitators to healthy behavior change within the home.

Clinician adherence to childhood overweight and obesity recommendations by race/ethnicity of the child.
This study describes school-based health center (SBHC) providers’ adherence to obesity guidelines. Providers (n = 28) were from SBHCs in six states serving children 5–12 years of age. Body mass index percentage was documented on 73 percent of charts and blood pressure percentage on 30.5 percent. Providers accurately diagnosed 40 percent overweight and 49.3 percent obese children.

Advancing informed consent for vulnerable populations.
The authors suggest an innovative approach that uses low health-literacy communication strategies and visual aids to augment and potentially replace the traditional approach to informed consent. This approach involves: (1) the use of effective health communication and low-literacy techniques, (2) the use of visual aids and graphics to promote understanding and guide the reader toward key study concepts, and (3) careful attention to child dissenting behaviors.

This study reports on countywide and subpopulation changes in menu-labeling awareness (i.e., seeing calorie information) and use (i.e., using calorie information) before and after policy implementation. It found that awareness and use of calorie information among residents who ate at regulated chains increased significantly and across all demographic subgroups.

Evaluation of care management intensity and bariatric surgical weight loss.
This study examined the effect of pre- and postoperative care management on weight loss following bariatric surgery. It found no statistically significant associations between either preoperative or postoperative care management intensity and postoperative change in body mass index at year 1 or year 2. Results were limited by heterogeneity of care management across sites and an inability to assess adherence to care management programs.

The authors argue that the barriers faced by individual anorectic products belie larger regulatory and clinical challenges to their mainstream adoption, and contribute to the irony that despite American’s penchant for high rates of pharmaceutical use, obesity drugs have fared remarkably poorly in the market.

The relationship between pretreatment dietary composition and weight loss during a randomised trial of different diet approaches.
Identifying pretreatment dietary habits that are associated with weight-loss intervention outcomes could help guide individuals’ selection of weight-loss approach among competing options. This study concluded that selection of a weight-loss approach on the basis of pretreatment macronutrient intake is unlikely to improve weight outcomes at the end of a 1-year treatment.

Impact of electronic health record clinical decision support on the management of pediatric obesity.
The investigators assessed the impact of electronic health record-based clinical decision support in improving the diagnosis and management of pediatric obesity. They found a statistically significant increase in the diagnosis of overweight/obesity, scheduling of follow-up appointments, frequency of ordering recommended laboratory investigations, and assessment and counseling for nutrition and physical activity.

Impact of food support on food security and body weight among HIV antiretroviral therapy recipients in Honduras: a pilot intervention trial.
This study assessed the effect of food support on food insecurity and body weight in a population of persons living with HIV with high prevalence of overweight and obesity by implementing a pilot intervention trial in four government-run HIV clinics in Honduras. At 6 months, the probability of severe food insecurity decreased by 48.3 percent in the food support group, compared to 11.6 percent in the education-only group.

Prenatal exposure to traffic pollution: associations with reduced fetal growth and rapid infant weight gain.
The authors explored air pollution exposure implications for postnatal growth. They found that infants exposed to the highest quartile of neighborhood traffic density had lower fetal growth, more rapid 0- to 6-month weight-for-length gain, and higher odds of weight-for-length ≥95th percentile at 6 months. Neighborhood traffic density was additionally associated with an infant being in both the lowest quartile of fetal growth and the highest quartile of 0- to 6-month weight-for-length gain.

Recruitment and enrollment of African Americans and Caucasians in a health promotion trial for persons with serious mental illness.
The researchers examined overall recruitment into a trial of cardiovascular disease prevention among people with serious mental illness (SMI), including the racial differences in interest, enrollment, and potential barriers to participation in a behavioral weight loss intervention aimed at adults with SMI. It found that 9.6 percent fewer African Americans enrolled due to inability to complete initial data collection.

Trends in diabetes incidence among 7 million insured adults, 2006-2011: the SUPREME-DM project.
This study of diabetes trends found that among 7 million insured adults there was no significant increase between 2006 and 2010. The small increase that occurred from 2010 to 2011 coincided with a shift from fasting plasma glucose testing to use of the HbA1c assay. The increase in diabetes incidence rates was significant only among racial and ethnic minority groups and among very obese persons.

Bariatric surgery in minority patients before and after implementation of a centers of excellence program.
This study compared rates of bariatric surgery for minority Medicare vs. non-Medicare patients before and after the 2006 implementation of a national coverage decision (NCD) restricting Medicare patients to centers of excellence for bariatric surgery. It found that the proportion of nonwhite Medicare patients receiving this procedure dropped from 27.5 percent before the NCD to 25.9 percent afterwards.

Chronic use of opioid medications before and after bariatric surgery.
This study of 11,179 obese patients receiving bariatric surgery found that among the 933 who were using opioids chronically before bariatric surgery, 77 percent continued chronic opioid use in the year following surgery (excluding the first 30 days after surgery). The amount of chronic opioid use was greater postoperatively than preoperatively.
