Revisiting the obesity paradox in health care expenditures among adults with diabetes.
This AHRQ-authored study explored and examined an obesity paradox identified by recent studies which suggests that in people without diabetes mortality risk increases with weight and in people with diabetes mortality risk decreases with weight. The researchers assessed changes in the association between body mass index (BMI) and health care expenditures in populations with diabetes and without diabetes while controlling for confounding risk factors. The researchers found that there is no obesity paradox; it is the result of statistical biases, and the study concluded that obesity in people with diabetes does not save costs.

Explaining the obesity paradox in healthcare utilization among people with type 2 diabetes.
The authors sought to examine changes in the relationship between BMI and number of visits in diabetic vs nondiabetic populations, controlling for confounding risk factors. Using MEPS data, they found that the obesity paradox does not exist at the utilization level and is due to the presence of statistical biases such as confounding and reverse causation.

Effect of a culturally adapted behavioral intervention for Latino adults on weight loss over 2 years: a randomized clinical trial.
Identifying effective weight loss interventions for Latino adults at risk of diabetes is of critical public health importance. The purpose of this study was to determine whether a culturally adapted behavioral intervention for Latino adults was more effective than usual care for weight loss over 24 months. The investigators concluded that among Latino adults with high diabetes risk, a culturally adapted behavioral lifestyle intervention was effective for weight loss over 12 months but not 24 months.

Physical activity in adolescents and children and relationship to metabolic health.
The purpose of this review was to summarize recent developments relating to the role of physical activity in insulin resistance and increased metabolic health in children and adolescents. Recent findings confirm the role of physical activity in decreasing insulin resistance and metabolic syndrome, but the authors note that the current literature is limited by unstandardized research methods and definitions. Future research that addresses these issues in order to offer targeted physical activity interventions is recommended.

Blood pressure control and other quality of care metrics for patients with obesity and diabetes: a population-based cohort study.
In this observational population-based cohort study, the investigators sought to examine the relationship between the extent of obesity and the achievement of guideline-recommended blood pressure goals and other quality of care metrics among patients with diabetes. The investigators suggest that although the cardiovascular risk for patients with obesity and diabetes is greater than for non-obese patients with diabetes, they found that patients with obesity are even further behind in achieving blood pressure control.

Just living with obese family members increases your risk of type 2 diabetes.
Although diabetes has been associated with the incidence of obesity, many diabetes patients are not obese. In this paper the authors discuss how just living in a household with one or more obese biologically related family members is a major risk factor for diabetes, even after accounting for all the other traditional risk factors.

Rationale and study protocol for Unidas por la Vida (United for Life): a dyadic weight-loss intervention for high-risk Latina mothers and their adult daughters.
The intervention aims to improve health behaviors and promote weight loss in two at-risk members of the same family: mothers with type 2 diabetes and their overweight/obese adult daughters who are at risk for developing diabetes. Both the dyadic and individual lifestyle interventions are expected to produce greater weight loss at 6, 12, and 18 months than those in minimal intervention control group.

This research letter discusses the trends in cardiovascular health of US adults by income from 2005-2014. The investigators assessed national trends in cardiovascular risk factors by income level among adults aged 25 years and older using data from the National Health and Nutrition Examination Surveys, a nationally representative, multistage probability sample of the US population, across 5 periods: 2005 to 2006, 2007 to 2008, 2009 to 2010, 2011 to 2012, and 2013 to 2014.

The burden of obesity on diabetes in the United States: Medical Expenditure Panel Survey, 2008 to 2012.
The researchers analyzed the risk of developing diabetes and the annual cost of diabetes for a US general population. Their results suggested that the annual health care expenditure differentials between those with and without diabetes of age 50 years were the highest for individuals with class II ($12,907) and class III ($9,703) obesity.

Text message support for weight loss in patients with prediabetes: a randomized clinical trial.
This study explores whether text message support enhances weight loss in patients offered Diabetes Prevention Program (DPP) classes. Stratification by language demonstrated a significant treatment effect in Spanish speakers but not in English speakers. It concluded that text message support can lead to clinically significant weight loss in patients with prediabetes.

Moderate and vigorous intensity exercise during pregnancy and gestational weight gain in women with gestational diabetes.
The researchers estimated the associations of exercise intensity during pregnancy with the rate of gestational weight gain (GWG) from gestational diabetes (GDM) diagnosis to delivery. Upon stratification by prepregnancy overweight/obesity, they found that significant associations were only observed for BMI greater than or equal to 25.0. There were no associations observed for moderate intensity exercise.

Statins and new-onset diabetes mellitus and diabetic complications: a retrospective cohort study of US healthy adults.
The authors studied the long-term relationship between statin use and new-onset diabetes among Tricare beneficiaries. They found that diabetes, diabetic complications, and overweight/obesity were more commonly diagnosed among statin-users than similar nonusers in a healthy cohort of adults, demonstrating that short-term clinical trials might not fully describe the risk/benefit of long-term statin use for primary prevention.

Racial/ethnic differences in gestational diabetes prevalence and contribution of common risk factors.
This study aimed to assess racial/ethnic differences in relative contribution of risk factors of gestational diabetes mellitus (GDM) among Asian subgroups (Asian Indian, Chinese, Filipino, Japanese, Korean, and Vietnamese), Hispanics, non-Hispanic blacks, and non-Hispanic whites. It found that GDM was most prevalent among Asian Indians (19.3 percent). Relative risks were similar across all race/ethnic groups.

Using an electronic activity monitor system as an intervention modality: a systematic review.
The authors aimed to synthesize the efficacy and feasibility results of electronic activity monitor systems (EAMSs) within published physical activity interventions. They found preliminary evidence suggesting that EAMS can increase physical activity and decrease weight significantly, but their efficacy compared to other interventions has not yet been demonstrated.

Impact of education on weight in newly diagnosed type 2 diabetes: every little bit helps.
The researchers sought to evaluate the benefit of participation in more limited counseling and/or education among individuals with newly diagnosed type 2 diabetes in more modest real-world clinical settings. The average weight loss of patients who received counseling/education alone during the follow-up period (up to three years post-exposure to participation) was 6.3 lbs., and, if received with medication prescription, 8.1 lbs. The weight loss associated with medication was only 3.5 lbs.

Trends in diabetes incidence among 7 million insured adults, 2006-2011: the SUPREME-DM project.
This study of diabetes trends found that among 7 million insured adults there was no significant increase between 2006 and 2010. The small increase that occurred from 2010 to 2011 coincided with a shift from fasting plasma glucose testing to use of the HbA1c assay. The increase in diabetes incidence rates was significant only among racial and ethnic minority groups and among very obese persons.

Dyadic collaboration in shared health behavior change: the effects of a randomized trial to test a lifestyle intervention for high-risk Latinas.
The authors sought to evaluate the feasibility of a pilot, dyad-based lifestyle intervention, the Unidas por la Vida program, for improving weight loss and dietary intake among high-risk Mexican American mothers with Type 2 diabetes and their overweight/obese adult daughters. They found that, at 16 weeks, Unidas participants lost significantly more weight compared with the control participants, and intervention participants also were more likely to be eating foods with lower glycemic load and less saturated fat. They concluded that interventions that draw upon multiple people who share a health-risk have the potential to foster significant changes in lifestyle behaviors and in social network members' health-related involvement.
