Integrating a patient engagement app into an electronic health record-enabled workflow using interoperability standards.
The authors sought to use interoperability standards to integrate the COVID-19 Tracker, a patient mobile application, with an EHR. Their clinical decision support integration project benefited from a standards-based approach, but they encountered challenges due to issues concerning implementation and experience of the standards-based application programming interface, Health Level 7 Fast Healthcare Interoperability Resources (FHIR) in the EHR. The authors concluded that FHIR standards may provide a promising mechanism for overcoming barriers in the integration of patient engagement apps with EHRs, but that expansion of available FHIR resources will improve workflow integration.

Patient and provider experience of electronic patient portals and secure messaging in mental health treatment.
This study explored patient and provider experience of patient electronic access to the mental health treatment record and the use of secure messaging. Participants received online surveys with questions about their experiences. Researchers concluded that the implementation of electronic access to mental health notes requires a transition from viewing the medical record as the exclusive tool of providers to that of a collaborative tool for patients and providers to achieve treatment goals.

A mixed-methods evaluation of standalone personal health record use by patients with type 2 diabetes.
The purpose of this study was to compare use of a standalone personal health records (PHRs) by patients with Type 2 diabetes to usual care through assessment of self-care behaviors, and short-term impact on social cognitive outcomes and hemoglobin A1c (HbA1c). Five themes emerged from the qualitative analysis describing participants' experiences with the PHR and identifying reasons for lack of engagement. Study findings revealed low PHR uptake and minimal impact on study outcomes, including lack of communication and information-sharing between patients and providers.

Evaluation of electronic health record-integrated digital health tools to engage hospitalized patients in discharge preparation.
Researchers sought to evaluate the effect of electronic health record (EHR)-integrated digital health tools comprised of a checklist and video on transitions-of-care outcomes for patients preparing for discharge. They found that EHR-integrated digital health tools to prepare patients for discharge did not significantly increase patient activation and was associated with a longer length of stay.

Using electronic health record portals to improve patient engagement: research priorities and best practices.
This literature review identified 53 studies published from September 2013 to June 2019 that informed best practices and priorities for future research on patient engagement with electronic health record (EHR) data through patient portals. While 90% of health care systems now offer patient portals, only 15-30% of patients use them to access their physicians or health care systems. Studies reviewed mostly involved outpatient settings and fell into 3 major categories: interventions to increase use of patient portals, usability testing of portal interfaces, and documentation of patient and clinician barriers to portal use. Limited health or digital literacy impacted patients’ use of portals. Clinicians reported a lack of workflows to support patient engagement through portals.

Acute care patient portal intervention: portal use and patient activation.
The aim of this study was to describe the use of an acute care patient portal and investigate its association with patient and care partner activation in the hospital setting. The investigators concluded that portal users most often accessed the portal to view their clinical information, though portal usage was limited to only the first few days of enrollment. They found an association between the use of the portal and HIT tools with improved levels of patient activation.

Patient-centered engagement and symptom/toxicity monitoring in the new era of tumor next-generation sequencing and immunotherapy: the OncoTool and OncoPRO platforms.
This paper describes the development of the OncoTool and OncoPRO platforms to help patients with late-stage cancer (stages III-IV) and their providers in providing patient-centered education and remote and routine monitoring of symptoms and toxicities after tumor next-generation sequencing testing and treatment. The OncoTool is a web-based educational resource tailored for people with advanced cancer. It aims to provide patients with easy-to-understand treatment options and associated toxicities as well as evidence-based strategies for managing symptoms and improving stress management. It is fully integrated with OncoPRO which provides feedback on patient-reported outcomes (PROs) to clinicians. The data from the platform can be integrated with the patient’s electronic health record (HER) and can provide an alert message. These systems are currently being tested with 4 trials – 1 for OncoTool and the other 3 for OncoPRO.

Patient and family engagement during treatment decisions in an ICU: a discourse analysis of the electronic health record.
This study looked at patient and family engagement during treatment decisions with patients in a medical ICU. A total of 52 patients were included who had been admitted to a hospital ICU during 2016. Half of them met a consensus definition of chronic critical illness, and the other half either died or had transitioned to hospice care in the ICU. Clinical electronic health records (EHRs) were used to document decision points. The phrase “goals of care” was frequently found in the HER and was used to indicate poor prognosis.

Parental perceptions of displayed patient data in a PICU: an example of unintentional empowerment.
The objective of this study was to explore the perceptions of parents of pediatric patients in a PICU regarding real-time open electronic health record data displayed in patient rooms. The investigators suggest that a new health information technology system providing continuous access to open electronic health record data may be an effective way to empower and engage parents in the PICU, but also note potential drawbacks.

Disparities in patient-reported interest in web-based patient portals: survey at an urban academic safety-net hospital.
Offering hospitalized patients' enrollment into a health system's patient portal may improve patient experience and engagement throughout the care continuum, especially across care transitions, but this process is less studied than portal engagement in the ambulatory setting. The objective of this study was to evaluate sociodemographic characteristics associated with interest in a health care system's portal among hospitalized patients and reasons for no interest.

Engaging hospitalized patients with personalized health information: a randomized trial of an inpatient portal.
This study examined the effects of an inpatient portal intervention on patient activation, patient satisfaction, patient engagement with health information, and 30-day hospital admissions. A randomized trial was conducted from March 2014 to May 2017 with 426 English- or Spanish-speaking patients from 2 cardiac medical-surgical units at an urban academic medical center. Patients were randomized into 3 groups: 1) usual care, 2) tablet with general Internet access, and 3) tablet with an inpatient portal. There was a difference in patient activation between the 3 groups, but the inpatient portal group had lower 30-day hospital admissions. There was also a difference with patient engagement with health information between the inpatient portal and tablet-only groups.

Engaging hospital patients in the medication reconciliation process using tablet computers.
Researchers conducted a pilot study to determine whether patients’ use of an electronic home medication review tool on a table computer could improve medication safety before or after hospitalization. Patients were randomized to the tool and out of 76 patients approached, 65 participated. About three-quarters (74%) made changes to their home medication list. Out of that total, 74% of the changes identified had a significant or greater potential severity, and 49% had a greater than 50-50 chance of harm. This medication reconciliation tool showed great potential to improve medication safety during and after hospitalization.

Understanding secure messaging in the inpatient environment: a new avenue for communication and patient engagement.
Patient portals, and the secure messaging feature in particular, have been studied in the outpatient setting, but research in the inpatient setting is relatively less mature. In this study, the investigators analyzed and categorized messages sent within an inpatient portal to understand the topics discussed in secure messaging in the inpatient environment. They concluded that their analysis of secure message content suggested certain message types and topics such as Alerts/Requests and Questions about symptoms and treatment plans were particularly important to patients.

Patient portal usage in pediatric urology: is it meaningful use for everyone?
This study examined pediatric urology patient portal enrollment and activation patterns at a tertiary pediatric hospital in Southern California by race/ethnicity, preferred language, gender, and residential region. The study concluded that primary language and socioeconomic factors may be significant barriers to portal adoption. Patient education to reduce these barriers may increase portal acceptance and increase meaningfulness to the portal for patients/parents and providers.

Keystrokes, mouse clicks, and gazing at the computer: how physician interaction with the EHR affects patient participation.
The purpose of this study was to investigate whether the different ways physicians interact with the computer vary in their effects on patient participation in the consultation, physicians' efforts to facilitate patient involvement, and silence. The investigators found that patients may be more reluctant to actively participate in medical encounters when physicians are more physically engaged with the computer (e.g., keyboard activity) than when their behavior is less demonstrative (e.g., gazing at EHR).

Patient perceptions of receiving test results via online portals: a mixed-methods study.
The researchers conducted a mixed-methods study to explore patients' experiences and preferences when accessing their test results via online portals. They found that nearly two-thirds (63 percent) did not receive any explanatory information or test result interpretation at the time they received the result. Patients experienced negative emotions often with abnormal results, but sometimes even with normal results.

This study sought to determine if electronic health record (EHR) tools and patient engagement can improve the quality of chronic kidney disease (CKD) care. It found that, among high-risk patients, those in the intervention arm were significantly more likely to have an office visit with a nephrologist compared with those in the control arm.

Usability evaluation of a commercial inpatient portal.
The authors aimed to understand how users interact with, learn to use, and communicate with their providers through an inpatient portal. They found that participants frequently made operational errors in navigation and assuming non-existent functionalities; participants' learning styles varied, with age as a potential factor that influenced how they learned MyChart Bedside; and participants preferred to message providers individually and wanted feedback on status. They concluded that, for inpatient portals to be effective in promoting patient engagement, it remains critical for technology developers and hospital administrators to understand how users interact with this technology and the resources that may be necessary to support its use.

Understanding views on everyday use of personal health information: insights from community dwelling older adults.
As a first step in formulating the role of personal health information management (PHIM) in healthy aging, researchers explored the perspectives of older adults on health and health information used in their everyday lives. Participants expressed wellness from a position of personal strength by focusing on wellness activities for staying healthy through: (1) personal health practices, (2) social network support, and (3) residential community engagement.

In this study, the investigators explored patient perspectives on program outreach messages containing content tailored to EHR-derived diabetes risk factors--a theory-driven strategy to increase the persuasiveness of health communications.

Nurse, patient, and care partner perceptions of a personalized safety plan screensaver.
A patient safety plan dashboard was developed that captures disparate data from the electronic health record that is then displayed as a personalized bedside screensaver. End user perceptions of the content and interface of the personalized safety plan screensavers were identified and strategies to overcome the barriers to use for future iterations were defined. Differences emerged stemming from each group of end users' role on the care team.

Provider use of a novel EHR display in the pediatric intensive care unit. Large Customizable Interactive Monitor (LCIM).
The purpose of this study was to explore providers' perspectives on the use of a novel technology, "Large Customizable Interactive Monitor" (LCIM), a novel application of the electronic health record system implemented in a Pediatric Intensive Care Unit. The study revealed this next generation HIT might have great potential for family-centered rounds, team education during rounds, and family education/engagement in their child's health in the patient room.

International perspectives on sharing clinical data with patients.
The researchers investigated the current state of approaches for providing patients with access to their own clinical information. Participants span 16 countries across six continents, and the authors found that there is considerable variability in the level of maturity, the degree of government involvement, the technical infrastructure, and the plans for future development across the world. They concluded that we are still in the early stages of deploying patient engagement technologies and have yet to identify optimal strategies in this arena.
