Letter by Radecki regarding article, "safety of thrombolysis in stroke mimics: results from a multicenter cohort study".
In this letter commenting on an article on the treatment of stroke mimics, the author asserts that the difficult question of the acceptable rate of misdiagnosis remains. He suggests that patients undergoing thrombolytic therapy for acute ischemic stroke have confirmatory testing such as an MRI with diffusion-weighted sequences and that the incidence of neuroimaging negative events be reported.

Long-term results of a randomized controlled trial of a nonoperative strategy (watchful waiting) for men with minimally symptomatic inguinal hernias.
This study of 254 men with minimally symptomatic inguinal hernia who were assigned to watchful waiting (WW) found that WW is a reasonable and safe strategy. However, the study which followed these patients for up to 11.5 years found that symptoms usually progressed and an operation was eventually needed, with 79 percent of men older than 65 and 62 percent of younger men receiving surgical repair.

Traumatic intracranial injury in intoxicated patients with minor head trauma.
This study to estimate the prevalence of intracranial injury following minor head injury found that 8% of 225 intoxicated patients with minor head injury had clinically important injuries requiring either hospital admission or neurosurgical followup. Neither the Canadian CT Head Rule nor NEXUS criteria had adequate sensitivity to be used with these patients.

Making difficult decisions: the role of quality of care in choosing a nursing home.
The authors investigated how quality of care affects nursing home choice. They examined choices in California, Ohio, New York, and Texas, in 2001. They found that, in all states, consumers were more likely to choose nursing homes of high hotel services quality but not clinical care quality; choice was also significantly associated with shorter distance from prior residence, not-for-profit status, and larger facility size.

A proposed 'health literate care model' would constitute a systems approach to improving patients' engagement in care.
The researchers proposed a Health Literate Care Model that would weave health literacy strategies into the widely adopted Care Model (formerly known as the Chronic Care Model). Their new model calls for approaching all patients with the assumption that they are at risk of not understanding their health conditions or how to deal with them, and then confirming and ensuring patients' understanding. They suggested that health literacy would then become an organizational value infused into all aspects of planning and operations. They also proposed a measurement framework to track the impact of the new Health Literate Care Model on patient outcomes and quality of care.

Incorporating patient and family preferences into evidence-based medicine.
The authors contend that the goal of patient-centered evidence based medicine (EBM) can be achieved through the incorporation of patient-derived data. Their article discusses the data needed to expand the EBM paradigm, including the role of patients’ families and their contributions to diagnosis seeking and treatment-related decisionmaking.
