Patient-reported complications related to peripherally inserted central catheters: a multicentre prospective cohort study.
The objective of this study was to systematically elicit and characterize peripherally inserted central catheter-related complications as experienced by patients during and after hospitalization. Results showed that over 60% of patients reported signs or symptoms of a possible complication or adverse effect after peripherally inserted central catheter placement. Bothersome complications from the patient perspective were more common than those that typically rise to the level of healthcare provider attention or concern. Recommendations included an understanding of the patient experience in order to provide safe and effective care.

Perspectives on central-line-associated bloodstream infection surveillance in home infusion therapy.
This study examined in the differences between 3 professional society members who provide home infusion services in diagnosing and defining central-line-associated bloodstream infections (CLABSIs). The three societies surveyed were members of the Infusion Nurses Society (INS), the National Home Infusion Association (NHIA), and the Society for Healthcare Epidemiology of America Research Network (SRN). The INS is a 6000-member global organization of main nurses who work in all practice settings including home infusion. The NHIA is a 400-member trade organization focused on providing infusion products and services in the home, and SRN is a network of 111 healthcare institutions that collaborate on research to prevent healthcare-associated infections and antibiotic resistance. There was a difference in the criteria used for defining a CLABSI with home testing, and their use of the Association for Professionals in Infection Control/Healthcare Infection Control Practices Advisory Committee (APIC-HICPAC) criteria. Although some perspective was gained from a wide variety of professionals, there was a low response rate, which suggests the possibility of response bias.

The impact of chlorhexidine bathing on hospital-acquired bloodstream infections: a systematic review and meta-analysis.
This review and meta-analysis examined the effect of chlorhexidine gluconate (CHG) bathing on reducing hospital-acquired bloodstream infections (HABSIs) and also assessed fidelity to the implementation of this intervention. The literature research included randomized controlled trials, cluster randomized trials, and quasi-experimental studies that evaluated the effect of CHG bathing versus a non-CHG comparator; studies involving pediatric patients, pre-surgical CHG use, and those without a non-CHG comparison arm were excluded. Outcomes were HABSIs, patient-centered outcomes, and implementation fidelity assessed through five elements: adherence, exposure or dose, quality of delivery, participant responsiveness, and program differentiation. The findings indicate that patient bathing with CHG significantly reduced the incidence of HABSIs in both ICU and non-ICU settings. Many studies, however, did not report fidelity to intervention or patient-centered outcomes.

Effectiveness of a multistate quality improvement campaign in reducing risk of surgical site infections following hip and knee arthroplasty.
The authors assessed the effect of a multistate quality improvement campaign to promote the adoption of evidence-based surgical site infection (SSI) prevention practices. Rates of SSI among Medicare beneficiaries undergoing hip and knee arthroplasty during pre-intervention and post-intervention in five states included in a multistate trial of the Project JOINTS campaign and five matched comparison states were analyzed. The authors found a larger reduction of SSI rates following hip and knee arthroplasty in intervention states than in the matched control states.

Beyond entry and exit: Hand hygiene at the bedside.
This study assessed compliance with, knowledge of, and attitudes toward the World Health Organization (WHO) 5 moments for hand hygiene (HH) using a modified WHO HH observation form and a survey that assessed health care personnel (HCP) knowledge, opinions, and barriers to HH. Of the 218 HCPs who completed the survey, less than one-third were familiar with the WHO 5 moments and only 21& of that group could recall the 5 moments. 302 HH opportunities in 104 unique HCP-patient interactions were observed, but with infrequent compliance. The researchers conclude that lack of recognition of opportunities at the bedside and for glove use may contribute to low compliance.

Integrating bedside nurses into antibiotic stewardship: a practical approach.
This study looked into a framework for nurses to integrate antibiotic stewardship (AS) into their clinical work with patients. The practices that nurses can take include improving antibiotic prescribing practices through appropriate obtainment of Cloistridioides difficile tests, appropriate urine culturing practices, optimal antibiotic administration, accurate and detailed documentation of antibiotic allergy histories, and through the prompting of antibiotic time outs. Barriers were also identified to engagement of nurses in AS and offered potential solutions.

Association between value-based incentive programs and catheter-associated urinary tract infection rates in the critical care setting.
This letter discussed a study which was done on value-based incentive programs to reduce the number of catheter-associated urinary tract infections (CAUTI) in intensive care units (ICUs). The study used data from 592 hospitals in the District of Columbia and 49 states. Researchers found these incentive programs did not significantly reduce CAUTI.

Comparison of hospital surgical site infection rates and rankings using claims versus National Healthcare Safety Network surveillance data.
This article examines national policies that target healthcare-associated infections by use of medical claims and National Healthcare Safety Network surveillance data. The authors looked at rates and rankings for surgical site infection following colon surgery in 155 hospitals, and found low concordance between these two data sources; they conclude that this underscores the limitations of evaluating hospital quality by using claims data.

Less lumens-less risk: a pilot intervention to increase the use of single-lumen peripherally inserted central catheters.
To reduce risk of complications, existing guidelines recommend use of peripherally inserted central catheters (PICCs) with the minimal number of lumens. This recommendation, however, is difficult to implement in practice. The investigators conducted a pilot study to increase the use of single-lumen PICCs in hospitalized patients. The authors indicated that implementing a single-lumen PICC default and providing education and indications for multilumen devices improved PICC appropriateness.

Development and validation of a clinical prediction rule to predict transmission of methicillin-resistant Staphylococcus aureus in nursing homes.
This study’s goal was to develop and validate a clinical prediction rule that can predict transmission of methicillin-resistant Staphylococcus aureus (MRSA) in nursing homes. The researchers wanted to identify residents who were most likely to transmit MRSA to health-care workers (HCWs) on their hands or clothing during clinical care. Demographic and clinical characteristic data was used from residents of community nursing homes in Maryland and Michigan from 2012 to 2014. The clinical prediction rule that was developed was then externally validated in a cohort of Department of Veterans Affairs nursing home residents from 7 states between 2012 and 2016. Variables in the prediction model included sex, race, resident dependency on care, the presence of any medical device, diabetes mellitus, and chronic skin breakdown. The prediction model showed good performance although it showed less utility in the validation cohort.

Michigan Appropriate Perioperative (MAP) criteria for urinary catheter use in common general and orthopaedic surgeries: results obtained using the RAND/UCLA Appropriateness Method.
Using the RAND Corporation/University of California Los Angeles (RAND/UCLA) Appropriateness Method, the authors assessed the appropriateness of indwelling urinary catheter placement and different timings of catheter removal for routine general and orthopaedic surgery procedures. The investigators found that appropriateness of catheter use varied by procedure, accounting for procedure-specific risks as well as expected procedure time and intravenous fluids. Procedural appropriateness ratings for catheters were summarised for clinical use into three groups, described in detail in the paper.

Quality improvement in ambulatory surgery centers: a major national effort aimed at reducing infections and other surgical complications.
Researchers recruited ambulatory surgery centers (ASCs) to implement and then modify patient safety practices that were used in hospitals, but may not be appropriate for ASCs. They recruited 665 ASCs in 47 US states and had them provide suggestions for modifying safe practices with the use of a surgical safety checklist and infection control practices.

A new frontier: central line-associated bloodstream infection surveillance in home infusion therapy.
As more home infusion agencies consider ways to perform surveillance for central line–associated bloodstream infection (CLABSI), an understanding of the assorted challenges is necessary. The authors discussed these challenges, which include a lack of a widely accepted standard definition of CLABSIs in home infusion therapy, the lack of a reporting platform, the absence of a reporting requirement, and barriers in obtaining the needed information to identify the presence of a potential CLABSI and adjudicate whether it meets a CLABSI definition. The 21st Century Cures Act will expand Medicare coverage for home infusion therapy services by 2021, likely leading to increased pressure for home infusion therapy CLABSI surveillance. Benchmarking of CLABSI data can usher in informed work to reduce CLABSIs and enhance patient safety in home infusion therapy.

Associations between enteral nutrition and acute respiratory infection among patients in New York metropolitan region pediatric long-term care facilities.
This study investigated the outcomes of children and a subsample of infants in pediatric long-term care facilities are need enteral nutrition (EN). Those patients who need EN had a higher risk of acute pediatric infection (ARI) and a lower odds of discharge than those who did not. Infants had a particular higher risk of comorbidities and infections when they were fed using percutaneous feeding tubes.

Peripherally inserted central catheters in the ICU: a retrospective study of adult medical patients in 52 hospitals.
This study quantified use of peripherally inserted central catheters in the ICU versus the general ward in 52 Michigan hospitals. Variation in complications and outcomes of use were compared. Use in the ICU produced worse outcomes than those inserted in the general ward.

Cost-effectiveness of pre-operative Staphylococcus aureus screening and decolonization.
In this study, the investigators developed a decision analytic model to evaluate the impact of a preoperative Staphylococcus aureus decolonization bundle on surgical site infections (SSIs), health-care-associated costs (HCACs), and deaths due to SSI. The investigators predict that the treat-all strategy would be the most effective and cost-saving strategy for preventing SSIs. However, they concluded that because this strategy might select more extensively for mupirocin-resistant S. aureus and cause more medication adverse effects than the test-and-treat approach or the SOC, additional studies are needed to define its comparative benefits and harms.

The purpose of this research was to compare gesture-function mappings for experts and novices using a 3D, vision-based, gestural input system when exposed to the same context of anesthesia tasks in the operating room (OR). Results showed that although domain expertise is influential when creating gesture-function mappings, both experts and novices should be able to use a gesture system intuitively, so development methods need to be refined for considering the needs of different user groups. Recommendations include the development of a touchless interface for perioperative anesthesia in order to reduce bacterial contamination.

Identification of surgical site infections using electronic health record data.
The objective of this study was to develop an algorithm for identifying surgical site infections (SSIs) using independent variables from electronic health record data and outcomes from the American College of Surgeons National Surgical Quality Improvement Program to supplement manual chart review. The investigators concluded that they identified a model that accurately identified SSIs. They indicated that the framework presented can be easily implemented by other American College of Surgeons National Surgical Quality Improvement Program-participating hospitals to develop models for enhancing surveillance of SSIs.

Factors for compliance with infection control practices in home healthcare: findings from a survey of nurses' knowledge and attitudes toward infection control.
This study analyzed survey responses from nurses at 2 large, certified home healthcare agencies to explore levels of compliance with infection control practices and identify associated demographic, knowledge, and attitudinal correlates. The findings suggested that efforts to improve compliance with infection control practices in home healthcare should focus on strategies to alter perceptions about infection risk and other attitudinal factors.

The aims of this qualitative study were to (a) explore perspectives on infection prevention among families visiting children in pediatric long-term care facilities and (b) identify facilitators of and barriers to optimal hand hygiene using semistructured in-depth interviews with 10 family members visiting two New York City metropolitan area facilities.

User testing an information foraging tool for ambulatory surgical site infection surveillance.
Surveillance for surgical site infections (SSIs) after ambulatory surgery in children requires a detailed manual chart review to assess criteria defined by the National Health and Safety Network. Electronic health records (EHRs) impose an inefficient search process. Using text mining and business intelligence software, the authors developed an information foraging application, the SSI Workbench, to visually present which postsurgical encounters included SSI-related terms and synonyms, antibiotic, and culture orders. This study compares the Workbench and EHR.
