Metformin use and health care utilization in patients with coexisting chronic obstructive pulmonary disease and diabetes mellitus.
This study aimed at investigating the effect of metformin on health care utilizations in patients with coexisting chronic obstructive pulmonary disease (COPD) and diabetes mellitus (DM). The authors concluded that the use of metformin in patients with coexisting COPD and DM was associated with fewer COPD-specific ER visits and hospitalizations, especially in low-complexity COPD.

Characteristics of desaturation and respiratory rate in postoperative patients breathing room air versus supplemental oxygen: are they different?
Routine monitoring of postoperative patients with pulse oximetry-based surveillance monitoring has been shown to reduce adverse events. However, there is some concern that pulse oximetry is limited in its ability to detect deterioration quickly enough to allow for intervention in patients receiving supplemental oxygen. To address such concerns, this study expands on the current limited knowledge of differences in desaturation and respiratory rate characteristics between patients breathing room air and those receiving supplemental oxygen.

Identification of perioperative pulmonary aspiration in children using quality assurance and hospital administrative billing data.
This study aims to identify the incidence of and risk factors for perioperative aspiration in children using quality assurance data supplemented by administrative billing records, and to examine the utility of billing data as a supplementary data source. The investigators found that International Classification of Diseases, Ninth Revision codes for aspiration used as a secondary data source were nonspecific for perioperative aspiration, but when combined with record review yielded a 30% increase in identified cases of aspiration over quality assurance data alone.

Outcomes after endoscopic dilation of laryngotracheal stenosis: an analysis of ACS-NSQIP.
Endoscopic management of pediatric subglottic stenosis (SGS) is common, however no multi-institutional studies have assessed its perioperative outcomes. This study examined outcomes after endoscopic dilation of laryngotracheal stenosis. The study found that open airway reconstruction is associated with longer length of stay and increased reintubations and reoperations, suggesting a possible opportunity to improve value in healthcare in the appropriately selected patient. Reoperations and readmissions following endoscopic dilation are more prevalent in children less than one year.

Reduced risk of acute exacerbation of COPD after bariatric surgery: a self-controlled case series study.
Little is known about the impact of weight reduction on chronic obstructive pulmonary disease (COPD)-related outcomes in patients who are obese. This study found that the risk of an ED visit or hospitalization for acute exacerbation of COPD substantially decreased after bariatric surgery in patients who are obese. This observation suggests the effectiveness of substantial weight reduction on COPD morbidity.

The association of aspirin use with severity of acute exacerbation of chronic obstructive pulmonary disease: a retrospective cohort study.
The researchers hypothesized that, in patients hospitalized for acute exacerbation of COPD (AECOPD), long-term aspirin use is associated with lower risks of disease severity (in-hospital death, mechanical ventilation use, and hospital length-of-stay). They found that in patients with AECOPD, aspirin use was associated with lower rates of in-hospital mortality and invasive mechanical ventilation use, and shorter length-of-stay.

Mobile critical care recovery program (m-CCRP) for acute respiratory failure survivors: study protocol for a randomized controlled trial.
The Mobile Critical Care Recovery Program (m-CCRP) study is a two arm, randomized clinical trial. The researchers will randomize 620 patients admitted to the ICU with acute respiratory failure requiring mechanical ventilation to one of two arms - m-CCRP intervention versus attention control. Their primary aim is to assess the efficacy of m-CCRP in improving the quality of life of acute respiratory failure survivors at 12 months

Clinical impact of external laryngeal manipulation during laryngoscopy on tracheal intubation success in critically ill children.
In this retrospective observational study, the objective was to evaluate the association between external laryngeal manipulation use and initial tracheal intubation attempt success in pediatric ICUs. The investigators found that external laryngeal manipulation during direct laryngoscopy was associated with lower initial tracheal intubation attempt success in critically ill children, even after adjusting for underlying differences in patient factors and provider levels. They asserted that the indiscriminate use of external laryngeal manipulation cannot be recommended.

Interobserver reliability of the Berlin ARDS definition and strategies to improve the reliability of ARDS diagnosis.
This study sought to examine the interobserver reliability of the Berlin acute respiratory distress syndrome (ARDS) definition and examine strategies for improving the reliability of ARDS diagnosis. It concluded that the reliability of the Berlin ARDS definition is moderate. Combining independent reviews by multiple clinicians or improving methods to identify bilateral infiltrates on chest imaging are important strategies for improving the reliability of ARDS diagnosis.

Obesity and severity of acute exacerbation of chronic obstructive pulmonary disease.
A study was done to determine if there was an increase in severity and mortality with chronic obstructive pulmonary disease (COPD) patients who were also obese. Researchers used data from 2012-2013 State Inpatient Databases data of seven U.S. states. 17% of COPD patients in the database were defined as obese. Obesity was shown to increase use of ventilation, increased hospital stays but not in-hospital mortality.

The association of antidepressant treatment with COPD maintenance medication use and adherence in a comorbid Medicare population: a longitudinal cohort study.
Researchers examined the association between antidepressant treatment and use of and adherence to chronic obstructive pulmonary disease (COPD) maintenance medications among patients with new-onset COPD and comorbid depression. They concluded that regularly treated depression may increase use of and adherence to necessary maintenance medications for COPD.

On the relative role of different age groups during epidemics associated with respiratory syncytial virus.
This study evaluated the roles played by individuals in different age groups during respiratory syncytial virus (RSV) epidemics in the United States between 2001 and 2012, using the previously defined relative risk (RR) statistic estimated from the hospitalization data from the Healthcare Cost and Utilization Project. Its estimates suggest the prominent relative roles of children aged </=10 years (particularly among those aged 3-6 years) in propagating RSV epidemics.

Frequency of desaturation and association with hemodynamic adverse events during tracheal intubations in PICUs.
In this retrospective cohort study, the investigators aimed to determine the occurrence and severity of desaturation during tracheal intubations and the association with adverse hemodynamic tracheal intubation-associated events. They found that moderate and severe desaturation were reported among 19% and 13% of all tracheal intubation encounters; Moderate and severe desaturations were independently associated with the occurrence of adverse hemodynamic events.

How often is sinus surgery performed for chronic rhinosinusitis with versus without nasal polyps?
The purpose of this study was to compare the rate of surgical interventions for chronic rhinosinusitis with nasal polyposis (CRSwNP) and chronic rhinosinusitis without nasal polyposis (CRSsNP). The investigators found that almost 30% of endoscopic sinus surgeries were performed for CRSwNP, and these cases were, on average, more extensive, used more OR time, and more often used image guidance than surgeries for CRSsNP.

Mortality and pulmonary outcomes of extremely preterm infants exposed to antenatal corticosteroids.
This study sought to determine if exposure to antenatal corticosteroids is associated with a lower rate of death and pulmonary morbidities by 36 weeks' postmenstrual age. It concluded that among infants 22-28 weeks' gestational age, any or partial antenatal exposure to corticosteroids compared to no exposure is associated with a lower rate of death while the rate of bronchopulmonary dysplasia in survivors did not differ.

Interobserver agreement in the evaluation of B-lines using bedside ultrasound.
The researchers evaluated agreement among trained emergency physicians assessing the degree of B-line presence on bedside ultrasound in patients presenting to the emergency department (ED) with acute undifferentiated dyspnea. They concluded that interrater agreement was best in the anterior/superior thoracic zones followed by the lateral/superior zones for both expert/expert and expert/novice pairs.

Physiologic findings in children previously ventilator dependent at home due to bronchopulmonary dysplasia.
Bronchopulmonary dysplasia (BPD) is the primary respiratory complication of premature birth. Little is known about spirometric indices in patients with severe BPD who were previously ventilator dependent at home. The study concluded that extreme prematurity associated with severe BPD requiring home ventilator support carries significant risks of morbidity. These patients had substantially diminished respiratory function reflecting airflow abnormalities that remained static over time.

Incidence and complications of interstitial lung disease in users of tocilizumab, rituximab, abatacept and anti-tumor necrosis factor alpha agents, a retrospective cohort study.
Interstitial lung disease (ILD) is a common extra-articular condition in rheumatoid arthritis (RA), but few studies have systematically investigated its incidence and risk factors in patients receiving anti-tumor necrosis factor-alpha (anti-TNFα) agents or alternate mechanisms of action (MOAs). After examining 13,795 episodes of biologic exposure in 11,219 patients, researchers found no significant differences in the risk of ILD and its related complications between RA patients receiving anti-TNFα agents and those receiving alternate MOA agents.

Multimorbidity and COPD Medication receipt among Medicaid beneficiaries with newly diagnosed COPD.
The aim of this study was to examine the association between multimorbidity and chronic obstructive pulmonary disease ( COPD) medication receipt among Medicaid beneficiaries with newly diagnosed COPD. It found that in this group 81.9 percen had at least one co-occurring chronic condition. After controlling for subject characteristics, adults with multimorbidity were less likely to receive COPD medications compared with those without any inflammation-related multimorbidity.

Cross-immunity between strains explains the dynamical pattern of paramyxoviruses.
The researchers used long-term incidence data on Respiratory Syncytial Virus (RSV), three serotypes of Human Parainfluenza Virus (HPIV), and Human Metapneumovirus to study mathematical models of different mechanisms of pathogen interaction. Their results showed a strong signal of cross-protection from RSV in controlling the timing and magnitude of HPIV outbreaks, and a stronger interaction with more closely related serotypes.

Risk factors for acute kidney injury during aminoglycoside therapy in patients with cystic fibrosis.
The aim of this study was to identify factors associated with acute kidney injury (AKI) during intravenous aminoglycoside (AG) courses in this population. It identified receipt of an AG within 90 days prior to admission, longer duration of AG therapy, low serum albumin, and receipt of trimethoprim/ sulfamethoxazole as independent risk factors for developing AKI.

Glucocorticoids and hospital length of stay for children with anaphylaxis: a retrospective study.
The objective of this study was to evaluate whether glucocorticoid administration is associated with improved outcomes in children with anaphylaxis. The investigators found that the use of glucocorticoids was inversely associated with prolonged length of stay among children hospitalized with anaphylaxis, but was not associated with 3-day emergency department revisits among discharged children, thus supporting the use of glucocorticoids in children hospitalized with anaphylaxis.

Symptom burden of chronic lung disease compared with lung cancer at time of referral for palliative care consultation.
The authors compared the symptom burden of chronic lung disease with that of lung cancer at the time of initial palliative care consultation. They found that patients with chronic lung disease have symptom burdens similar to those of patients with lung cancer at the time of the first palliative care encounter. They concluded that, given the population burden of chronic lung disease and limitations in the palliative care workforce, attention should be focused on ensuring that pulmonologists are prepared to assess and manage the common palliative care needs of patients with chronic lung disease.

Trends in tracheostomy for mechanically ventilated patients in the United States, 1993-2012.
This study investigated trends in tracheostomy use, timing, and outcomes in the United States. It found that over the past two decades, tracheostomy use rose substantially in the United States until 2008, when use began to decline. In addition, there was an observed dramatic increase in discharge of tracheostomy patients to long-term care facilities.
