Lifestyle strategies after intentional weight loss: results from the MAINTAIN-pc randomized trial.
The aim of this study was to explore the strategies related with successful long-term weight loss maintenance. Researchers analyzed data from the 24-month Maintaining Activity and Nutrition Through Technology-Assisted Innovation in Primary Care (MAINTAIN-pc) trial. MAINTAIN-pc recruited 194 adults with recent intentional weight loss and randomized participants a group using tracking tools plus coaching (i.e., coaching group) or tracking tools without coaching (i.e., tracking-only group). The participants reported the lifestyle strategies they utilized in the previous 6 months, including self-monitoring, group support, behavioral skills, and professional support. The study found that at baseline, 100% used behavioral skills, 73% used group support, 69% used self-monitoring, and 68% used professional support in the past 6 months; at 24 months, the rates were 98%, 60%, 75%, and 61%, respectively. The number of participants using individual strategies did not vary significantly over time, but the overall number of strategies participants reported decreased. A greater number of strategies were utilized at baseline and 6 months compared to 12- and 24-month follow-ups. The coaching group utilized a greater number of strategies at months 6 and 12 than the tracking-only group. Consistent utilization of professional support strategies over the 24-month study period was related with less weight regain.

Reach, acceptability, and perceived success of a telehealth diabetes prevention program among racially and ethnically diverse patients with gestational diabetes: the gem cluster-randomized trial.
The purpose of this study was to describe overall and race/ethnicity-specific reach, acceptability, and perceived success from an effective telehealth diabetes prevention lifestyle program for patients with gestational diabetes mellitus, implemented in the Gestational Diabetes Effects on Moms (GEM) cluster-randomized controlled trial. An intervention of 13 telephone sessions and behavior change techniques (BCTs) in a healthcare system were tested by GEM. Survey respondents largely rated BCTs as very helpful. Black and White respondents reported more limited success reaching a healthy weight than Asian/Pacific Islander, Hispanic, and multiracial/other women. The researchers concluded that a telehealth diabetes prevention lifestyle program showed both reach and acceptability across racial/ethnic groups, and that similar interventions could encourage preventive care access and help reduce disparities in the risk for diabetes.

An international, mixed-methods study of the perceived intrusiveness of remote digital diabetes monitoring.
This paper describes an international online vignette-based survey to assess the relationship between remote digital monitoring (RDM) modalities for diabetes and intrusiveness in patients’ lives. The survey was conducted from February-June 2019 in 30 countries with 1010 participants. A total of 2680 vignette-assessments were completed with 52% on type 1 diabetes. Increased intrusiveness was associated with food monitoring compared with glucose- and PA-monitoring alone and permanent monitoring with real-time physician-generated feedback compared with monitoring for a week with feedback. Public-sector data handling was associated with decreased intrusiveness as compared with the private sector.

Effect of electronic health record-based coaching on weight maintenance: a randomized trial.
This study compared long-term weight regain after participation in a weight loss management program using an electronic health record (EHR)-based weight maintenance intervention program. Participants were adult outpatients with a BMI of 25 kg/m2 or higher, had intentional weight loss of at least 5% in the previous 2 years, and had no bariatric procedures in the previous 5 years. The EHR tools included weight, diet and physical activity tracking sheets, standardized surveys and reminders. Patients were randomly assigned to the coaching or non-coaching group. They were tracked for 24 months with 24 scheduled contacts. Results showed patients who used the EHR tools plus coaching had less weight regain than patients using EHR tools alone.

Real-time demonstration of a mHealth app designed to reduce college students hazardous drinking.
The authors developed a smartphone application (SmarTrek) that targets college students and aims to reduce risky alcohol use. They found that the majority of participants agreed that SmarTrek was easy to use and that the information provided was useful and had a positive effect on decreasing their drinking.

Identifying and planning for individualized change: patient-provider collaboration using lightweight food diaries in healthy eating and irritable bowel syndrome.
This study examined if the use of a photo-based food diary app will help patients with irritable bowel syndrome (IBS) help gather food and symptom data to help their doctors with disease management. The investigators designed and developed Foodprint which includes a mobile and web app and pre-visit note to go to experts which communicate an individual’s expectations and questions. A control group and a group with IBS were used to determine the effectiveness of Foodprint.

A tailored letter based on electronic health record data improves gestational weight gain among women with gestational diabetes mellitus: the Gestational Diabetes' Effects on Moms (GEM) cluster-randomized controlled trial.
The purpose of this study was to evaluate whether a tailored letter improved gestational weight gain (GWG) and whether GWG mediated a multicomponent intervention's effect on postpartum weight retention among women with gestational diabetes mellitus (GDM). The authors concluded that a tailored electronic health record-based letter improved GWG, which mediated the effect of a multicomponent intervention in reducing postpartum weight retention.

Online lifestyle modification intervention: survey of primary care providers' attitudes and views.
The aim of this study was to evaluate the response of primary care providers (PCPs) to a referral model for implementing a year-long online intervention for weight loss to obese adult patients. The investigators suggest that understanding providers' views and barriers regarding the integration of online tools will facilitate widespread implementation of an online lifestyle modification intervention.

Leveraging EHRs for patient engagement: perspectives on tailored program outreach.
Electronic health records (EHRs) present healthcare delivery systems with scalable, cost-effective opportunities to promote lifestyle programs among patients at high risk for type 2 diabetes, yet little consensus exists on strategies to enhance patient engagement. In this study, the investigators explored patient perspectives on program outreach messages containing content tailored to EHR-derived diabetes risk factors--a theory-driven strategy to increase the persuasiveness of health communications.

When personal tracking becomes social: examining the use of Instagram for healthy eating.
The researchers aimed to inform the design of tools to support healthy behaviors by understanding how people appropriate Instagram to track and share food data, the benefits they obtain from doing so, and the challenges they encounter. They concluded that participants tracked to support themselves and others in their pursuit of healthy eating goals.

Reconsidering the device in the drawer: lapses as a design opportunity in personal informatics.
The authors used a survey of 141 people who had lapsed in using Fitbit and identified three use patterns and four perspectives on tracking. Participant Fitbit use and perspective on tracking influenced the preference of those surveyed, and the authors surfaced these preferences in a series of contrasts. They concluded by discussing how these contrasts suggest new designs and opportunities in other tracking domains.

Beyond abandonment to next steps: understanding and designing for life after personal informatics tool use.
The authors extended recent research with new insights drawn from people reflecting on their experiences after they stop tracking, examining how designs continue to influence people even after abandonment. They connected their findings to models of personal informatics and identified six reasons why people stop tracking and five perspectives on life after tracking. In this article, they discussed these results and opportunities for design to consider life after self-tracking.

Electronic documentation of lifestyle counseling and glycemic control in patients with diabetes.
The aim of this study was to establish quantitative characteristics of documentation of lifestyle counseling that are associated with improved glycemic control in patients with diabetes, using a previously validated natural language processing system that enables abstraction of lifestyle counseling documentation from narrative electronic provider notes. It identified novel quantitative characteristics of electronic documentation of lifestyle counseling that are associated with improved glycemic control in patients with diabetes.

Collaborative practice improvement for childhood obesity in rural clinics: the Healthy Eating Active Living Telehealth Community of Practice (HEALTH COP).
The authors assessed the impact of participation in a virtual quality improvement (QI) learning network on adherence to clinical guidelines for childhood obesity prevention in rural clinics. They found that children who received care from clinicians who led the implementation of the intervention at their clinic showed significant improvements in nutrition and physical activity. Virtual QI learning networks in geographically dispersed clinics can significantly increase clinicians' adherence to guidelines for childhood obesity and improve access to recommended care for rural and underserved children.

Following the money: copy-paste of lifestyle counseling documentation and provider billing.
To find out whether copied documentation of lifestyle counseling was used to justify higher evaluation and management (E&M) charges, researchers investigated 12,527 patient encounters in which lifestyle counseling was documented. When time spent on counseling was recorded, copied lifestyle counseling was associated with a decrease of 70.5% in the odds of higher E&M charges.
