InfoSAGE: Supporting elders and families through online family networks.
With an increasingly elderly population, families are finding it increasingly challenging to coordinate care for their older family members. This paper reports on the findings of InfoSAGE, an online private social network that has tools for communication and care coordination for elders and their families.

Information sharing across generations and environments (InfoSAGE): study design and methodology protocol.
This open prospective cohort study aimed to assess a novel, Internet based, family-centric communication and collaboration platform created to address the information needs of elders and their informal caregivers in a community setting. It used a mixed methods approach, utilizing qualitative survey data along with website usage analytic data.

Understanding secure messaging in the inpatient environment: a new avenue for communication and patient engagement.
Patient portals, and the secure messaging feature in particular, have been studied in the outpatient setting, but research in the inpatient setting is relatively less mature. In this study, the investigators analyzed and categorized messages sent within an inpatient portal to understand the topics discussed in secure messaging in the inpatient environment. They concluded that their analysis of secure message content suggested certain message types and topics such as Alerts/Requests and Questions about symptoms and treatment plans were particularly important to patients.

Training to optimize collaborative use of an inpatient portal.
Inpatient portals are an emerging technology that can facilitate collaborative interactions between patients and care teams, but little is known about how organizations attempt to implement and facilitate inpatient portal use for their providers. The investigators found that interviews with 220 care team members and 4 IT staff identified 3 new areas of training needed to optimize inpatient portal implementation and use that extend beyond training to use other technologies.

Expanding the Facebook platform to engage and educate online communities.
This article discusses the development and use of a mobile application (app) called Liver Space that was developed through Facebook’s platform. This app is for the pediatric liver community including patients and caregivers. Unlike most Facebook health groups, this one is vetted by healthcare providers who are specialists. The app provides up-to-date information and includes emerging news, summaries from important scholarly journals and human interest stories. There is an “ask an expert” function incorporated into Liver Space. Also included in the app is the ability for users to track their labs and weight and to graph the data.

Keystrokes, mouse clicks, and gazing at the computer: how physician interaction with the EHR affects patient participation.
The purpose of this study was to investigate whether the different ways physicians interact with the computer vary in their effects on patient participation in the consultation, physicians' efforts to facilitate patient involvement, and silence. The investigators found that patients may be more reluctant to actively participate in medical encounters when physicians are more physically engaged with the computer (e.g., keyboard activity) than when their behavior is less demonstrative (e.g., gazing at EHR).

Designing asynchronous communication tools for optimization of patient-clinician coordination.
The authors elicited narratives about patients' experiences and expectations for using asynchronous communication to address medical issues with their clinicians. They then presented opportunities for designing asynchronous communication tools to better facilitate understanding of and coordination around care activities between patients and clinicians.

Understanding patients' health and technology attitudes for tailoring self-management interventions.
The researchers elicited attitudes toward technology use and participation in care of 40 participants in a maximum variation sample. They found three participant clusters-"Proactive Techies," "Indie Self-Managers," and "Remind Me! Non-Techies"-that represent varying attitudes toward health behaviors and technologies that were independent of race, education level, and age. Their approach provides insight into how people prioritize important values related to health participation and technology.

Information sharing preferences of older patients and their families.
This study identified how patients older than 75 years (hereinafter, elders) and family caregivers of such patients approach sharing of health information, with the hope of applying the results to collaborative patient portals. It found that information sharing and control are complex issues even under the most well-meaning circumstances. While elders may delegate control and share information with family, they want to retain granular control of their information.

Parent preferences for communicating with their adolescent's provider using new technologies.
The researchers assessed the views of parents of adolescents on various means to communicate with their adolescents’ physicians about vaccine reminders and appointments, medication refills and test results including phone, mail, e-mail, text messages, and personal health records (PHR). They found that young parents of adolescents are most likely to accept text message vaccine reminders; suburban parents prefer e-mail reminders. Personal health records are accepted generally by those parents who are younger and suburban.

More than telemonitoring: Health provider use and nonuse of life-log data in irritable bowel syndrome and weight management.
The researchers sought to develop an in-depth understanding of providers’ facilitators and barriers to successfully integrating life-log data into their practices and creating better experiences. Providers reported using self-monitoring data to enhance provider-patient communication, develop personalized treatment plans, and to motivate and educate patients, in addition to using them as diagnostic and adherence tools. Barriers included a lack of time to review detailed records and questions about providers' expertise to review it.

Design of a cluster-randomized trial of electronic health record-based tools to address overweight and obesity in primary care.
The researchers described the design of a trial to examine the effectiveness of electronic health record-based tools for the assessment and management of overweight and obesity among adult primary care patients, as well as the challenges encountered. New features included reminders to measure height and weight, and an alert asking providers to add overweight or obesity to the problem list.

Patient satisfaction and provider use of electronic communication: a cross-sectional analysis.
This cross sectional analysis examined provider patterns of communication with patients outside of the clinic setting via cellphone, email and text messaging and the relationship between communication behaviors and patient satisfaction. Among other conclusions, the investigators found that providers who made their email addresses available to patients had significantly higher overall satisfaction scores than those who did not, although there were no statistically significant differences in individual satisfaction domains.

Patterns of response to patient-centered decision support through a personal health record.
The investigators evaluated patients' patterns of responses to notifications regarding guideline-recommended services delivered through a personalized health record (PHR). They found that approximately 61% of participants accessed the PHR or received the care that triggered the message after the first message and 73% after the first two messages. They concluded that, in this low-intensity intervention, participants accessed the PHR and received recommended care.

Patient-to-physician messaging: volume nearly tripled as more patients joined system, but per capita rate plateaued.
The authors investigated trends in secure e-mail messaging between physicians and patients. Analyzing messages in a large academic health system's patient portal from 2001-2010, they found that physicians in the aggregate saw a near tripling of e-mail messages during the study period. However, the number stabilized between 2005 and 2010, at an average of 18.9 messages.

Predictors and intensity of online access to electronic medical records among patients with cancer.
This study set out to determine predictors and patterns of use of a Web-based portal for accessing PHRs and communicating with health providers among patients with cancer. It found that among patients with cancer, PHR portal use is frequent and increasing. Younger patients, white patients, and patients with upper aerodigestive malignancies exhibit the heaviest portal use.

Patient engagement in the inpatient setting: a systematic review.
The purpose of this review article is to summarize the existing scientific literature regarding patient engagement during inpatient care. Its focus is on use of health information technology to increase patient engagement and self-efficacy in this setting. The authors conclude that there are considerable gaps in knowledge regarding patient engagement in the hospital and inconsistent use of terminology regarding patient engagement.
