Introducing a patient portal and electronic tablets to inpatient care.
The researchers implemented a pilot inpatient portal (MyChart Bedside [Epic Systems]) using Android tablets to provide patients and their families and caregivers with an expected care plan each day, a roster of the care team, the ability to exchange secure messages with the care team, a way to write and save notes, and access to health information. They conducted the 90-day pilot program in 15 rooms on 1 patient unit in each of 2 hospitals and gave tablets to 179 patients. They found that patients wanted immediate access to laboratory results, that patients admitted for 1-2 days found the tablets less important, that those readmitted insisted on having a tablet, and that those with their own electronic devices were less likely to accept the tablet.

Design a learning-oriented fall event reporting system based on Kirkpatrick model.
Patient fall has been a severe problem in healthcare facilities around the world due to its prevalence and cost. Routine fall prevention training programs are not as effective as expected. Using event reporting systems is the trend for reducing patient safety events such as falls, although some limitations of the systems exist at current stage. The authors of this paper summarized these limitations through literature review, and developed an improved web-based fall event reporting system.

Reducing symptom distress in patients with advanced cancer using an e-alert system for caregivers: Pooled analysis of two randomized clinical trials.
The aim of this paper was to assess the effects on cancer patient symptom distress of an eHealth system that alerts clinicians to significant changes in the patient's symptoms, as reported by a family caregiver. It found that when severe caregiver-reported symptoms were shared with clinicians, the symptoms were more likely to be subsequently reported as improved than when the symptoms were not shared with clinicians.

Patient portal use and experience among older adults: systematic review.
The aim of this review was to assess the existing research landscape related to patient portal and electronic personal health records (ePHRs) use and experience among older adults and to understand the benefits and barriers to older adults' use and adoption of patient portals and ePHRs. Overall, it found 2 main barriers to use: (1) privacy and security and (2) access to and ability to use technology and the Internet.

Building stronger online communities through the creation of Facebook-integrated health applications.
Social media, such as Facebook, provides a powerful mechanism to connect individuals with similar diseases, but current platforms do not achieve their full potential to help patients communicate with one another or with the medical community. The authors of this viewpoint article believe that an opportunity exists for health care professionals to strengthen online communities by creating apps that use the Facebook platform or a programming interface. Development of such apps are discussed, with an eye toward the needs of the patient, parent, or caregiver as end-users and involving their input.

Legal, practical, and ethical considerations for making online patient portals accessible for all.
The authors argue that there are potential legal mandates for improving portal accessibility (e.g., the Civil Rights and the Rehabilitation Acts), as well as ethical considerations to prevent the exacerbation of existing health and health care disparities. To address these legal, practical, and ethical considerations, they present standards and broad recommendations that could greatly improve the reach and impact of portal Web sites.

Framework to guide the collection and use of patient-reported outcome measures in the learning healthcare system.
Web-based collection of patient-reported outcome measures (PROMs) in clinical practice is expanding rapidly as electronic health records include web portals for patients to report standardized assessments of their symptoms. As the value of PROMs in patient care expands, a framework to guide the implementation planning, collection, and use of PROs to serve multiple goals and stakeholders is needed. In this study, researchers identified diverse clinical, quality, and research settings where PROMs have been successfully integrated into care and routinely collected and analyzed drivers of successful implementation.

The researchers conducted a chart review in a comprehensive EMR with a patient portal and results management features but found no associations between patient characteristics, diabetes risk factors, or provider characteristics and follow-up failures.

Multi-institutional randomized trial testing the utility of an interactive three-dimensional contouring atlas among radiation oncology residents.
The present study reports on the efficacy and usability of a web-based contouring atlas compared with those of existing contouring resources in a randomized trial. Compared with the residents using currently available resources, the residents using eContour had improved contour agreement with both the consensus and the expert contours for the high-risk clinical target volume and greater agreement with the expert contour for the contralateral parotid gland.

Physician perception of the role of the patient portal in pediatric health.
Little information regarding physician opinion of the patient portal is available, with almost no information gathered in the pediatric environment. Using a mixed-methods approach, physicians in a large pediatric medical facility and integrated delivery network were surveyed by online quantitative questionnaire and structured interviews. Physicians reported the portal's role in more communication efficiency for patients, parents, and providers.

A multidisciplinary approach to designing and evaluating electronic medical record portal messages that support patient self-care.
The authors describe a project intended to improve the use of Electronic Medical Record (EMR) patient portal information by older adults with diverse numeracy and literacy abilities, so that portals can better support patient-centered care. Their approach combines quantitative measures, as well as experimental and individual difference methods in order to investigate which formats are more effective, and whether some formats benefit some types of patients more than others.

BMT roadmap: a user-centered design health information technology tool to promote patient-centered care in pediatric HCT.
“BMT Roadmap" is a web-based application that integrates patient-specific information and includes several domains. In this study, BMT Roadmap was provided to 10 caregivers of patients undergoing first-time hematopoietic cell transplant (HCT) . Caregivers found the tool useful and easy-to-use, leading them to want even greater access to information. BMT Roadmap was feasible, with no disruption to inpatient care.

The economic case for digital interventions for eating disorders among United States college students.
This article aimed to estimate the costs, in United States (US) dollars, of a stepped care model for online prevention and treatment among US college students to inform meaningful decisions regarding resource allocation and adoption of efficient care delivery models for EDs on college campuses. A stepped care model was estimated to achieve modest cost savings compared to standard care, but these estimates need to be tested with sensitivity analyses.

A federally qualified health center (FQHC) sought to help patients interpret their records by embedding context-specific hyperlinks to plain-language patient education materials in its portal. Black patients, Latino patients comfortable using English, and patients covered by Medicaid were more likely to use the informational hyperlinks than other patients.

Increasing consumer engagement by tailoring a public reporting website on the quality of diabetes care: a qualitative study.
The goal of this study was to determine if tailoring quality reports to persons with diabetes mellitus and co-occurring chronic conditions would increase user engagement with a website that publicly reports the quality of diabetes care. It concluded that tailoring can be used to improve public reporting sites for individuals with chronic conditions, ultimately allowing consumers to make more informed health care decisions.

Care to Plan: an online tool that offers tailored support to dementia caregivers.
This study developed and tested the feasibility of "Care to Plan" (CtP), an online resource for dementia caregivers (e.g., relatives or unpaid nonrelatives) that generates tailored support recommendations. It concluded that, based on the integrated qualitative and quantitative data, CtP was simple and easy to understand. The streamlined visual layout facilitated utility and the individualized recommendations could meet the needs of users.

Correlates of online health information-seeking behaviors in a low-income Hispanic community.
The authors explored the correlates of online health information-seeking behaviors among Hispanic residents of a low-income urban neighborhood. They found that age and Spanish as preferred language were negatively associated with online health information-seeking, whereas education and health literacy were positively associated. They concluded that their findings have implications for designing online health information resources and interventions appropriate for the populations they are likely to reach.

Evaluation of a statewide HIV-HCV-STD online clinical education program by healthcare providers - a comparison of nursing and other disciplines.
The authors evaluated the New York State HIV-HCV-STD Clinical Education Initiative (CEI) online education program and compared the self-reported measures by clinicians from different disciplines. They found that physicians and nurse practitioners were the most satisfied, while pharmacists and case/care managers recorded lower than average responses. They recommended that online education programs consider the unique needs by clinicians from specific disciplines.

Health insurance information-seeking behaviors among the uninsured.
In order to better understand how to reach the uninsured and support their health insurance decision making, this study examined where the uninsured collect information about health insurance and the extent to which they trust those sources and media. Its findings suggest that strategies that pair health care professionals, lay health advisors, or community liaisons with the ubiquity of the Internet may be a strong approach for delivering quality health insurance information to the uninsured.

Increase in cancer center staff effort related to electronic patient portal use.
The authors examined patterns of use of electronic portals by clinic staff at a National Cancer Institute-designated comprehensive cancer center. They found that from 2011 to 2014, staff portal activity increased approximately 10-fold, and sixteen percent of all actions occurred outside clinic hours. They concluded that cancer center employee use of an electronic patient portal has increased markedly over time, particularly among nursing staff.

"I want to keep the personal relationship with my doctor": Understanding barriers to portal use among African Americans and Latinos.
The investigators sought to understand specific barriers to portal use among African American and Latino patients at Kaiser Permanente, which has had a portal in place for over a decade. Their findings suggest that uniform adoption of portal use across diverse patient groups requires more usable, more personalized websites, which may be particularly important for reducing health care disparities.

Association of a web-based handoff tool with rates of medical errors.
The researchers implemented a web-based handoff tool and training for health care professionals and evaluated the association of the tool with rates of medical errors in adult medical and surgical patients. They found that implementation of this tool was associated with a significant reduction in rates of medical errors, driven largely by a reduction in errors attributable to communication failure and errors that occurred during end-of-shift handoffs.
