The Telemedicine for Patients With Inflammatory Bowel Disease (TELE-IBD) clinical trial: qualitative assessment of participants' perceptions.
This qualitative clinical trial examined the use of telemedicine for managing patients with inflammatory bowel disease (IBD). The study called TELE-IBD, enrolled 348 patients, with 259 completing the study. Patients completing the study were interviewed after and results were positive in improving quality of life (QOL) by improving patient education and self-management.

Adding social determinants in the electronic health record in clinical care in Hawai'i: supporting community-clinical linkages in patient care.
Given its distinctive history, culture, and location, Hawai'i has unique social factors impacting population health. Local health systems are striving to address these issues to meet their patients' health needs. Yet the evidence on precisely how health care systems and communities may work together to achieve these goals are limited both generally and specifically in the Hawai'i context. This article described real-world efforts by 3 local health care delivery systems that integrated the identification of social needs into clinical care using the electronic health record (EHR).

Evaluation of multidisciplinary collaboration in pediatric trauma care using EHR data.
This study electronic health record (EHR) data to compare usage patterns from pediatric trauma patients with minor injuries at a Level I pediatric trauma center. The data was used to compare demographics, clinical and network characteristics, and emergency department (ED) length of stay (LOS). Three distinct groups were compared: fully connected, partially connected, and disconnected. The fully connected group had a decreased ED LOS compared with the partially connected group.

Systems engineering and human factors support of a system of novel EHR-integrated tools to prevent harm in the hospital.
This study examined systems engineering and human factors support of a system of novel electronic health record (EHR)-integrated tools for patient safety in the hospital. The authors established a Patient Safety Learning Laboratory of 2 core and 3 individual project teams to introduce a suite of digital health tools integrated with their EHR to identify, assess, and mitigate threats to patient safety. They identified 7 themes regarding use of 12 systems engineering and human factors over the 4-year project.

Adapting patient experience data collection processes for lower literacy patient populations using tablets at the point of care.
This study compared the acceptability of low-literacy tablet-based and traditional paper-based patient experience surveys in English and Spanish. The Consumer Assessment of Healthcare Providers & Systems Clinician & Group Survey (CG-CAHPS) was adapted for a pilot survey using low-literacy questions in Spanish and English. The majority of interview participants preferred the tablet version over the traditional paper-based survey. This was especially true for the younger and Latino respondents.

Advancing the use of patient-reported outcomes in practice: understanding challenges, opportunities, and the potential of health information technology.
The purpose of this study was to understand current opportunities and challenges with the use of patient-reported outcomes (PROs) and the potential for health information technology (HIT) to advance their use. Potential research areas to address PRO implementation challenges emerged, including measures harmonization; implementation process and workflow; electronic data collection and integration; and user-friendly data displays. Results showed that opportunities exist during different stages of PRO implementation to advance the use of PROs in clinical practice, with HIT being utilized to address challenges in data collection, integration, and visualization to make PRO data accessible and understandable to patients and providers.

Building consensus toward a national nursing home information technology maturity model.
This study describes the development of a nursing home information technology (IT) maturity model in 2 phases. The first phase was to develop a preliminary maturity model. Phase II involved 3 rounds of questionnaires using a Delphi panel of expert nursing home administrators. Three Delphi rounds were done, and this evolved the model from 5-stages (stages 1-5) to a 7-stage model (stages 0-6).

Clinician perspectives on electronic health records, communication, and patient safety across diverse medical oncology practices.
This study examined the effects of electronic health records (EHRs) on communication and patient safety in oncology practices. The authors conducted a survey of 297 oncology nurses and prescribers in a statewide collaborative. They found there was an inverse relationship between reliance on EHRs and safety.

Impact of a health information technology tool addressing information needs of caregivers of adult and pediatric hematopoietic stem cell transplantation patients.
The authors of this article developed BMT Roadmap, a health information technology application on a tablet, to address caregivers' unmet needs with patient-specific information from the electronic health record. They conducted a preliminary feasibility study of BMT Roadmap in caregivers of adult and pediatric hematopoietic stem cell transplantation (HSCT) patients and registered the study on ClinicalTrials.gov. They found that BMT Roadmap was a feasible intervention to implement in HSCT caregivers, associated with increased activation and decreased burden; quality of life, however, was lowered across hospitalization. The authors conclude that their findings support the need for further development of caregiver-specific self-directed resources and provision for both inpatient and outpatient across the HSCT trajectory.

Improving patient-provider communication and therapeutic practice through better integration of electronic health records in the exam room: a pilot study.
This study assessed the effectiveness of providing physicians who use electronic health records (EHRs) education for communication strategies and its impact on patient outcomes. The evidence-based Physician Asthma Care Education (PACE) program was extended in a pilot program using 18 providers and 126 adult patients with persistent asthma. Outcomes were assessed at baseline and 3- and 6-month postintervention intervals. Providers who completed the EHR-PACE program felt more confident in communicating with their patients, but there was no significant changes in patient asthma outcomes or their perceptions of their provider’s communication skills.

Non-English language availability of community health center websites.
This study examined the availability of the Health Resources and Services Administration (HRSA)-supported community health center (CHC) websites that provide translated home page content. Out of 1400 CHC homepages, 34.3% provided translated information with half using Google Translate. Odds of the homepage being translated increased as the limited English-proficient (LEP) population by county increased, Internet subscription at the state level increased, or if the CHC was in a metropolitan area. Most of the homepages were translated into Spanish in counties with higher Spanish LEP populations, but this was not true for other non-Spanish language LEP populations.

Novel health information technology to aid provider recognition and treatment of major depressive disorder and posttraumatic stress disorder in primary care.
This article describes a multicomponent health information technology screening tool designed to aid provider recognition and treatment of major depressive disorder and posttraumatic stress disorder (PTSD) in the primary care setting, with an eye toward meeting the mental health needs of traumatized refugees in the US Cambodian community. In a randomized controlled trial, 18 primary care providers were randomized to receive access to the mental health screening intervention, or to a minimal intervention control group. Cambodian American patients empaneled to participating providers were assigned to the providers' randomized group. From the results, the authors conclude that this approach offers the potential for training providers to diagnose and treat traumatized patients seeking mental health care in primary care.

Patient and family engagement during treatment decisions in an ICU: a discourse analysis of the electronic health record.
This study looked at patient and family engagement during treatment decisions with patients in a medical ICU. A total of 52 patients were included who had been admitted to a hospital ICU during 2016. Half of them met a consensus definition of chronic critical illness, and the other half either died or had transitioned to hospice care in the ICU. Clinical electronic health records (EHRs) were used to document decision points. The phrase “goals of care” was frequently found in the HER and was used to indicate poor prognosis.

The effect of administration mode on CAHPS survey response rates and results: a comparison of mail and web-based approaches.
The researchers compared response rates, respondents' characteristics, and substantive results for CAHPS surveys administered using web and mail protocols. They found that response rates to surveys administered using the Internet protocols were lower than for the surveys administered by mail, but characteristics of respondents and survey answers were very similar across protocols. Respondents without email addresses tended to be older, less educated, and more likely to be male than those with email addresses, and there were a few differences in their responses.

Perspectives of English, Chinese, and Spanish-speaking safety-net patients on clinician computer use: qualitative analysis.
The goal of this study was to understand how safety-net patients, including those with limited English proficiency, view clinician electronic health record (EHR) use. Through focus groups in English, Spanish, and Cantonese, results showed that linguistically diverse patients accepted the value of EHR use during outpatient visits but desired more eye contact, verbal warnings before EHR use, and screen-sharing. Support for clinicians in completing EHR-related tasks during the visit using patient-centered strategies for all patients is recommended.

Effect of restriction of the number of concurrently open records in an electronic health record on wrong-patient order errors: a randomized clinical trial.
This study assessed whether the belief that having only 1 electronic health record (EHR) open at a time as opposed to 4 will reduce the number of wrong-patient orders by clinicians. A randomized clinical trial was conducted with 3356 clinicians in a large New York Health system from October 2015 to April 2017. Outcomes from emergency department, inpatient, and outpatient settings showed that there seemed to be no difference in the number of wrong-patient order errors. However, most clinicians in the unrestricted group placed orders with a single-record open anyway which limited the power of the study.

Health information technologies in the support systems of pregnant women and their caregivers: mixed-methods study.
This study’s objective was to determine the number and nature of the components of the support systems of pregnant women and their caregivers (family members) and the role of health information technologies (HIT) in these support systems. Pregnant women and their caregivers were enrolled from advanced maternal-fetal and group prenatal care clinics. Participants filled out surveys which addressed stress levels, socioeconomic status, health literacy, and they drew a diagram of their support systems. Pregnant women overall had a larger support system than their caregivers. Their source of medical information was more likely to be specific medical information websites, while caregivers used more general internet search engines. No association was found between stress, support system size, and support system components.

Challenges and opportunities to improve the clinician experience reviewing electronic progress notes.
There is a need to understand better how clinicians review electronic notes and how note structure variability may impact clinicians' note-reviewing experiences. This article aimed to understand how physicians review electronic clinical notes and what impact section order has on note-reviewing patterns. The investigators indicated that their findings support the need to improve EHR note design and presentation to support optimal note review patterns for clinicians.

Parental perceptions of displayed patient data in a PICU: an example of unintentional empowerment.
The objective of this study was to explore the perceptions of parents of pediatric patients in a PICU regarding real-time open electronic health record data displayed in patient rooms. The investigators suggest that a new health information technology system providing continuous access to open electronic health record data may be an effective way to empower and engage parents in the PICU, but also note potential drawbacks.

Using health information technology to support use of patient-reported outcomes in rheumatology.
Using health information technology (IT) to capture information on patients’ experience of illness through patient-reported outcomes within rheumatology suggests that careful attention to human centered design, including detailed workflow planning, consideration of patient and physician burden, integration into the health IT ecosystem, and delivering information to the right person at the right time are all important. The authors recommended that technology applications be tested in diverse health systems and populations to ensure they are simple to interpret, useful for clinical decision-making, and effective in impacting outcomes.

Continuing patient care during electronic health record downtime.
This study examined the impact of electronic health record (EHR) downtime in hospitals on patient care. Two mid-Atlantic hospitals where the EHR system was either fully or partially unavailable were used to document the problems using historic performance data and semistructured interviews. A total of 17 hospital employees were interviewed. Laboratory test results were delayed an average of 62% during downtime events. Paper documentation created during the downtime period was often incomplete or incorrect. The authors provided recommendations to improve downtime contingency plans based on their findings.

Design and implementation of an interactive website for pediatric voice therapy-the concept of in-between care: a telehealth model.
The purpose of this article was to describe the design and implementation of a web-based portal that was developed to provide supported home practice between weekly voice therapy sessions delivered through telehealth to children with voice disorders. Participants and speech-language pathologists used the portal effectively, finding it useful for in-between care and reinforcement of therapeutic exercises. The authors add that a versatile individual, who is able to work with all project staff is essential to the development process.

Design and implementation of structured clinical documentation support tools for treating stroke patients.
This article describes the development of a customized structured clinical documentation toolkit that standardizes patient data collection to conform to Best Practices for treating patients with stroke. The toolkit collects demographic information for patients, relevant score test measures, and information on disability, treatment, and outcomes. Basic descriptive data gathered via the toolkits is provided and their utility in collecting patient data in a manner that supports both quality clinical care and research initiatives demonstrated. Quality improvement projects and current research initiatives using the toolkit are also discussed.

Improving response rates and representation of hard-to-reach groups in family experience surveys.
This study examined the use of an audio-enabled tablet to survey parents of children discharged from 4 units of a children’s hospital. Normal mail survey response rates are very low, especially for black, Latino, and low-income respondents. This survey was done day of discharge at the hospital and there was a response rate of 71.1% via tablet versus 16.3% for mail only. The Child Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey was used. Tablet response rates were highest with fathers, those more likely to have a high school education or less, less likely to be white, and more likely to be publicly insured. The results are promising for future surveys using tablet administration.
