Early seizures are predictive of worse health-related quality of life at follow-up after intracerebral hemorrhage.
Investigators tested the hypothesis that health-related quality of life at follow-up is different between patients with and without early seizures (and secondarily, with nonconvulsive status epilepticus) after intracerebral hemorrhage. Subjects were 33 patients whose quality of life was assessed 3 months after intracerebral hemorrhage onset. The researchers found that early seizures and nonconvulsive status epilepticus were associated with lower health-related quality of life at follow-up in survivors of intracerebral hemorrhage.

Minimal hepatic encephalopathy and mild cognitive impairment worsen quality of life in elderly patients with cirrhosis.
Researchers investigated the performance of elderly patients with cirrhosis on tests for minimal hepatic encephalopathy (MHE) and predementia mild cognitive impairment (MCI) and their effects on quality of life (QOL). They found that, in a multicenter study of elderly patients with cirrhosis and controls, the presence of MHE, regardless of MCI, was associated with poor cognition and QOL. They created adjusted norms that defined the high sensitivity of EncephalApp for the detection of MHE in older individuals and validated it in a separate cohort.

Sleep disturbances after pediatric traumatic brain injury: a systematic review of prevalence, risk factors, and association with recovery.
This is a systematic review to quantify sleep wake disturbances (SWD) after pediatric traumatic brain injury (TBI). These SWD can place children at risk for worse outcomes since sleep is needed for brain development and healing after injury. They also evaluated interventions for SWD and the association between SWD and other post-traumatic outcomes. Literature was searched from 1999-2019 evaluating sleep or fatigue in children hospitalized for TBI. Two independent reviewers assessed quality of the studies using the Newcastle-Ottowa Score for observational studies. Out of 966 identified articles, 126 full text articles were reviewed and 24 studies were included. Studies showed at least 20% of children with TBI had some degree of SWD including trouble falling or staying asleep, fatigue, daytime fatigue, and nightmares. SWD was negatively correlated with cognitive, behavioral, and quality of life outcomes. There was moderate-high risk of bias for all studies due to small sample size and lack of validated or objective SWD measures.

Trade-offs in quality-of-life assessment between the modified Rankin Scale and Neuro-QoL measures.
The objective of this study was to describe the physical and cognitive health of patients with differing levels of post-stroke disability as defined by modified Rankin Scale (mRS) scores. Cross-sectional correlations were also compared between the mRS and the Quality of Life in Neurological Disorders (Neuro-QoL) T-scores to longitudinal correlations of change estimates for each measure. A total of 745 patients with ischemic stroke (79%) or transient ischemic attack (21%) were enrolled in this study. There were larger differences observed in cognitive function for the severe mRS groups. Larger differences in physical function were observed in the mild-moderate mRS groups. These findings undermine the validity and utility as an outcome measure in longitudinal studies in ischemic stroke patients. But it is still efficient at capturing important differences in patient-reported quality of life, and is useful for identifying meaning cross-sectional differences among clinical subgroups.

Sleep measure validation in a pediatric neurocritical care acquired brain injury population.
Lingering morbidities including physical, cognitive, emotional, and psychosocial sequelae, termed the Post-Intensive Care Syndrome, persist years after pediatric neurocritical care (PNCC) hospitalization. Sleep disturbances impact other Post-Intensive Care Syndrome domains and are under-evaluated to date due to a lack of appropriate measurement tools. The present study evaluated the validity of the Sleep Disturbance Scale for Children (SDSC) to address the growing need for assessing sleep problems after PNCC.

Effect of collaborative dementia care via telephone and internet on quality of life, caregiver well-being, and health care use: the Care Ecosystem randomized clinical trial.
Few health systems have adopted effective dementia care management programs. The Care Ecosystem is a model for delivering care from centralized hubs across broad geographic areas to caregivers and persons with dementia (PWDs) independently of their health system affiliations. The purpose of this study was to determine whether the Care Ecosystem was effective in improving outcomes important to PWDs, their caregivers, and payers beyond those achieved with usual care.

Structured clinical documentation to improve quality and support practice-based research in headache.
The authors developed a proprietary toolkit to aid clinicians when creating clinical documentation in electronic medical records (EMRs). This toolkit will help clinicians provide discrete data and not unstructured free text which many clinicians enter into the EMR. The toolkit collects hundreds of fields of data and interprets score tests for a number of difference assessment tools for anxiety disorder, depression, migraine disability, and insomnia. The toolkit was used at 4346 initial patient visits as of April 1, 2018. The toolkit is being shared with other clinics as part of the Neurology Practice-Based Research Network.

Prophylactic seizure medication and health-related quality of life after intracerebral hemorrhage.
Using a prospective cohort study design, the investigators tested the hypothesis that prophylactic levetiracetam is independently associated with differences in cognitive function health-related quality of life. They found that prophylactic levetiracetam was independently associated with lower cognitive function health-related quality of life at follow-up after intracerebral hemorrhage.

Fever burden and health-related quality of life after intracerebral hemorrhage.
In this prospective observational cohort study, the investigators tested the hypothesis that increased burden of fever is independently associated with decreased health-related quality of life (HRQoL) at follow-up. The authors found that each additional day with a fever was predictive of worse HRQoL domains of Cognitive Function and Mobility after intracerebral hemorrhage up to 1 year. They suggest that HRQoL outcomes may be a sensitive and powerful way to measure the efficacy of fever control in future research.

HDQLIFE: the development of two new computer adaptive tests for use in Huntington disease, speech difficulties, and swallowing difficulties.
The authors developed a patient-reported outcome measure for use in the Huntington Disease Health-Related Quality of Life (HDQLIFE) Measurement System that focused on the impact that difficulties with speech and swallowing have on health-related quality of life in Huntington disease. These measures may have clinical utility in other populations where speech and swallowing difficulties are prevalent.

Hematoma locations predicting delirium symptoms after intracerebral hemorrhage.
The researchers tested the hypothesis that hematoma location is predictive of delirium symptoms in patients with intracerebral hemorrhage. Identifying 90 patients with intracerebral hemorrhage, they found that acute delirium symptoms were predicted by hematoma of right-hemisphere subcortical white matter and parahippocampal gyrus. The authors concluded that disruption of large-scale brain networks that normally support attention and conscious awareness was thus associated with acute delirium symptoms.
