Dashboards for visual display of patient safety data: a systematic review.
Methods to visualize patient safety data can support effective monitoring of safety events and discovery of trends. While quality dashboards are common, use and impact of dashboards to visualize patient safety event data remains poorly understood. The objective of this study was to understand development, use and direct or indirect impacts of patient safety dashboards. The investigators conducted a systematic review in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.

Complexity of the pediatric trauma care process: implications for multi-level awareness.
Trauma is the leading cause of disability and death in children and young adults in the US. While much is known about the medical aspects of inpatient pediatric trauma care, not much is known about the processes and roles involved in in-hospital care. Using human factors engineering (HFE) methods, the investigators combined interview, archival document and trauma registry data to describe how intra-hospital care transitions affect process and team complexity.

A comparison of two structured taxonomic strategies in capturing adverse events in U.S. hospitals.
The purpose of this study was to compare the ability of AHRQ’s Quality and Safety Review System (QSRS) and the proposed triadic structure for the 11th version of the International Classification of Disease (ICD-11) to capture adverse events in U.S. hospitals. One thousand admissions were chosen for this study in a random sample from 3 general, acute care hospitals in Maryland and Washington D.C. and abstracted by two different sets of coding specialists for QSRS and the ICD-11. The results showed less than 20 percent agreement between QSRS and ICD-11 in their ability to identify the same adverse events. The sensitivity of ICD-11 improved when coders were given a list of potential adverse events. The authors conclude that in its draft form, ICD-11 has a limited ability to capture the same types of events as QSRS and that coders may need more training to improve its ability to identify adverse events.

Developing pediatric patient safety research priorities.
This article is a commentary on a newly published article by Hoffman et al entitled “Priorities for Pediatric Safety Research”. The authors agree with the discussion in the article and further elaborate on several ideas in the article. The four points discussed are: 1) broadening the stakeholders; 2) recognize the role of distinct data sources; 3) understand real-world barriers to uptake of evidence-based strategies; and 4) incorporate maternal health factors into the research.

Neonatal intubation practice and outcomes: an international registry study.
Neonatal tracheal intubation is a critical but potentially dangerous procedure. In this study, the investigators sought to characterize intubation practice and outcomes in the NICU and delivery room (DR) settings and to identify potentially modifiable factors to improve neonatal intubation safety. They developed the National Emergency Airway Registry for Neonates and collected standardized data for patients, providers, practices, and outcomes of neonatal intubation. They suggest that their results will inform future interventional studies to improve neonatal intubation safety.

Analysis of errors in dictated clinical documents assisted by speech recognition software and professional transcriptionists.
The purpose of this study was to identify and analyze errors at each stage of the speech recognition (SR) assisted dictation process. The study concluded that seven in 100 words in SR-generated documents contain errors; many errors involve clinical information. That most errors were corrected before notes were signed demonstrates the importance of manual review, quality assurance, and auditing.

A statewide surgical coaching program provides opportunity for continuous professional development.
Researchers sought to develop and evaluate a video-based coaching program for board-eligible/certified surgeons. They found that, overall, participants were satisfied with their experience and found the coaching program valuable. Future research to evaluate the impact of coaching on practice change and patient outcomes is recommended.

Challenges in patient safety improvement research in the era of electronic health records.
The researchers used a case study involving a project on missed or delayed follow-up of test results to discuss real-world challenges in using electronic health records data for patient safety research. They suggested that many current data access and security policies and procedures must be rewritten and standardized across health care organization sin order to advance progress toward safer health care.

Ethical issues in patient safety research: a systematic review of the literature.
The authors provided a review of the literature related to ethics, oversight, and patient safety research; identified issues highlighted in articles as being of ethical relevance; described areas of consensus regarding how to respond to these ethical issues; and highlighted areas where additional ethical analysis and discussion are needed to provide guidance to those in the field.
