Recruitment challenges for low-risk health system intervention trials in older adults: a case study.
Researchers assessed factors associated with trial participation in the context of a low-risk intervention intended to reduce adverse drug events in recently hospitalized older adults. They found that recruiting older adults recently discharged from the hospital to participate in trials of low-risk, system-level interventions was challenging and may under-enroll the oldest individuals and those potentially at the highest risk for adverse events, limiting generalizability of study findings.

Screening for intimate partner violence, elder abuse, and abuse of vulnerable adults.
This Putting Prevention into Practice case study demonstrates how a U.S. Preventive Services Task Force (USPSTF) recommendation can used by clinicians. This case study uses the Screening for Intimate Partner Violence, Elder Abuse, and Abuse of Vulnerable Adults recommendation as an example. Three quiz questions are given which directly link to the recommendations. The questions involve screening guidelines, screening instruments, and follow-up if there is a positive screening and detailed answers are provided.

Acquisition of multidrug-resistant organisms in the absence of antimicrobials.
This nested case-control study was conducted among 137 nursing home residents who did not receive antimicrobials, with 44 acquiring a multi-drug resistant organism. Risk factors identified included receiving gastrointestinal medication that affected the gut microbiome, the number of visits from healthcare workers, pressure ulcers, and not residing in a dementia unit.

Early palliative care in advanced illness: do right by mama.
This letter describes a case study where the doctor and the family decided not to do aggressive treatment on their mother in her 80’s with metastatic lung cancer and pneumonia. After describing the intubation procedure and the use of mechanical ventilation, the family decided that palliative care was the best option. The doctor emphasizes the use of palliative care as the best outcome for many terminally ill patients.

Screening for cognitive impairment in older adults.
This case study describes a 72-year-old white man comes the physician’s office for a preventive visit. He does not have any symptoms of dementia but asks whether he should be screened. Based on the recommendations of the U.S. Preventive Services Task Force, the authors ask (and answer) three questions concerning approaches to screening for cognitive impairment, risk factors for cognitive impairment or dementia in older adults, and the epidemiology of cognitive impairment in the United States.

Interhospital transfers: the need for standards.
Patient transfers from one hospital to another are common and occur for a multitude of reasons with varied outcomes. The authors discuss interhospital transfers and difficulties encountered by the providers who care for these patients. They recommend further research to identify more clearly which patients are most likely to benefit from transfer and why.

This case study, based on recommendations of the U.S. Preventive Services Task Force, concerns a 65-year-old woman who presents for her yearly checkup. She walks one to two miles daily, is in generally good health, and has no significant cardiovascular history. Questions concern whether she should be screened for carotid artery stenosis, the accuracy of the different screening methods, and the benefits and harms of screening.
