Sleep disturbance in smokers with preserved pulmonary function and chronic obstructive pulmonary disease.
This study compared the magnitude and correlates of sleep disturbance between smokers with preserved pulmonary function and those with airflow obstruction. It found that among smokers with clinically identified chronic obstructive pulmonary disease (COPD), the severity of sleep disturbance is greater among those with preserved pulmonary function compared to those with airflow obstruction. Non-respiratory symptoms, such as depression, were associated with sleep disturbance in both groups.

Impact of comorbidities among Medicaid enrollees with chronic obstructive pulmonary disease, United States, 2009.
Researchers quantified the cost of Medicaid-insured patients with chronic obstructive pulmonary disease (COPD) co-diagnosed with other chronic disorders. They concluded that comorbidities markedly increased health services use among people with COPD insured with Medicaid, although ED visits in this study were predominantly unrelated to COPD. Acute care, hospital bed days, and total Medicaid-reimbursed costs increased as the number of comorbidities increased.

Chronic airway inflammation provides a unique environment for B cell activation and antibody production.
This study sought to determine the mechanisms that drive B cell activation and antibody production during chronic airway inflammation. It found that nasal polyps contained significantly elevated frequencies of plasmablasts, especially those that expressed the extrafollicular marker Epstein-Barr virus-induced protein 2 (EBI2), but significantly fewer germinal centre (GC) B cells compared with tonsil.

Proton pump inhibitors decrease eotaxin-3/CCL26 expression in patients with chronic rhinosinusitis with nasal polyps: possible role of the nongastric H,K-ATPase.
The researchers assessed the effect of type 2 mediators, particularly IL-13 and eotaxin-3, on tissue eosinophilia and disease severity in patients with chronic rhinosinusitis. The results demonstrated that proton pump inhibitors (PPIs) reduce IL-13-induced eotaxin-3 expression by airway epithelial cells. Furthermore, mechanistic studies suggest that the nongastric H,K-ATPase is necessary for IL-13-mediated epithelial responses.

A pilot study of symptom profiles from a polyp vs an eosinophilic-based classification of chronic rhinosinusitis.
Researchers hypothesized that appropriate chronic rhinosinusitis (CRS) subclassification would increase homogeneity of baseline symptoms, and identify characteristic symptoms of each subtype. They found that subclassifying CRS with symptoms alone is difficult with neither polyp status nor eosinophilia giving a distinctive clinical symptom profile. However, certain symptoms may help otolaryngologists identify CRS subtypes, which may help guide future treatments.

Screening for chronic obstructive pulmonary disease (COPD) in an urban HIV clinic: a pilot study.
This study aimed to determine the predictive value of a COPD screening strategy validated in the general population and to identify HIV-related factors associated with decreased lung function. It found that questionnaire and peak flow together had low sensitivity, but abnormal peak flow shows potential as a screening tool for COPD in HIV/AIDS. These data suggest that lung function may be influenced by HIV-related factors.

An official American Thoracic Society workshop report. A framework for addressing multimorbidity in clinical practice guidelines for pulmonary disease, critical illness, and sleep disorders.
The American Thoracic Society convened a workshop to establish a strategy to address multimorbidity within clinical practice guidelines. This report describes a framework that addresses multimorbidity in each of the key steps of guideline development: topic selection, panel composition, identifying clinical questions, searching for and synthesizing evidence, rating the quality of that evidence, summarizing benefits and harms, formulating recommendations, and rating the strength of the recommendations.
