Reliability and meaningful change of the Patient-Reported Outcomes Measurement Information System(®) Itch Questionnaire (PIQ) item banks in adults with atopic dermatitis.
The Patient-Reported Outcomes Measurement Information System Itch Questionnaire (PIQ), includes a suite of patient-reported outcome measures to assess the burden of itch in adults. In this study, the investigators sought to determine the smallest detectable change (SDC), threshold for meaningful improvement and test–retest reliability of PIQ item banks for assessing the burden of itch in adult AD.

The use of patient monitoring systems to improve sepsis recognition and outcomes: a systematic review.
This systematic review’s aim was to determine the impact of automated patient monitoring systems (PMSs) on sepsis recognition and outcomes. The review was conducted using articles published from 2008 through 2018. Nineteen articles were identified for inclusion with 4 systematic reviews and 15 individual studies. Study results for outcome measures were mixed, with more than half the studies showing a significant improvement in at least one outcome measure (eg mortality, intensive care unit length of stay, ICU transfer). Evidence for process measure improvement was of moderate strength across multiple hospital units.

Early stroke recognition and time-based emergency care performance metrics for intracerebral hemorrhage.
This study compared time for early stroke recognition for intracerebral hemorrhage for hospitals with and without stroke teams. An observational cohort study was conducted at an urban comprehensive stroke center from 2009 to 2017 with 204 cases included. Stroke team activation resulted in faster emergency care compared to no activation. This process resulted in shorter onset-to-arrival times, higher NIH Stroke Scale scores, and higher Glasgow Coma Scale scores.

Improving respiratory rate accuracy in the hospital: a quality improvement initiative.
Researchers initiated a quality improvement (QI) initiative in hospitals to improve respiratory rate measurement accuracy. Time-keeping devices were added to vital sign carts and patient care assistants were retrained on a newly modified workflow that included concomitant respiratory rate (RR) measurement during automated blood pressure measurement. The median RR measurement rate increased postintervention. This intervention was associated with a 7.8% reduced incidence of tachypnea-specific systemic inflammatory response syndrome. This QI initiative was interdisciplinary, low-cost, and low-tech.

Reduced cancer survival among adults with HIV and AIDS-defining illnesses despite no difference in cancer stage at diagnosis.
A cohort of persons with HIV (PWH) who had AIDS-defining illness (ADI) at the time of cancer diagnosis were compared to those without HIV and ADI with cancer diagnosis at the same stages. Those with ADI had a higher mortality rate (30-70%) with lung cancer statistically significant. This finding is related with HIV-related immune suppression.

A self-report version of the ways of responding: reliability and validity in a clinical sample.
This study evaluated the reliability and validity of a modified, self-report version of the Ways of Responding instrument measures (WOR-SR) in a community mental health sample with depressive symptoms. Subjects completed the WOR-SR and other measures of depressive symptoms, dysfunctional cognitions, functioning, quality of life, and interpersonal problems at multiple time points. The authors conclude from their results that the WOR-SR is a reliable and valid measure of compensatory skills in patients receiving treatment for depression at community mental health centers.

Application of the Third International Consensus Definitions for Sepsis (Sepsis-3) Classification: a retrospective population-based cohort study.
The Consensus Definitions present clinical criteria for the classification of patients with sepsis. Researchers investigated incidence and long-term outcomes of patients diagnosed with these classifications, which are currently unknown. Their findings support the use of the elevated sepsis-related organ failure assessment (SOFA) score from Sepsis-3, and elevated quick SOFA (qSOFA) score from Sepsis-3 classifications to identify patients with infection who are at elevated risk of poor outcomes.

Diagnostic testing and hospital outcomes of children with neurologic impairment and bacterial pneumonia.
This study assessed hospital-level variability in diagnostic testing and outcomes for children with neurologic impairment hospitalized with pneumonia. For children with neurologic impairment hospitalized with pneumonia, across hospital differences in diagnostic testing were not associated with clinically meaningful differences in outcomes. High-utilizing hospitals may be able to decrease diagnostic testing for children with neurologic impairment hospitalized with pneumonia without adversely impacting outcomes.

Yield of emergent neuroimaging in children with new-onset seizure and status epilepticus.
The purpose of this paper was to determine the yield of emergent neuroimaging among children with new-onset seizures presenting with status epilepticus. The authors found that a substantial minority of children with new-onset seizures presenting with status epilepticus have urgent or emergent intracranial pathology identified on neuroimaging and that magnetic resonance imaging is the preferred imaging modality when available and safe.

Association of early imaging for back pain with clinical outcomes in older adults.
This study compared function and pain at the 12-month follow-up visit among older adults who received early imaging with those who did not receive early imaging after a new primary care visit for back pain without radiculopathy. It found that early imaging was not associated with better 1-year outcomes. The researchers concluded that the value of early diagnostic imaging in older adults for back pain with radiculopathy is uncertain.
