Goals/Purpose:
The goal of this project was to create a patient-centered framework to analyze diagnostic breakdowns and apply it to large datasets to understand their incidence, types, and contributing factors in ambulatory care. Additionally, the project aimed to develop an EHR patient-portal approach to engage chronically ill patients in the diagnostic process, assessing safety, implementation, and stakeholder experiences at two US healthcare centers.
Specific Aims:
- Establish a new patient-centered framework co-designed with patients/families and care partners to measure and categorize patient-reported diagnostic breakdowns (PRDBs) and apply this new analytic tool to establish the incidence, types, and contributing factors to PRDBs in ambulatory care using two large and unique existing databases.
- Develop and implement a new EHR portal-based method enabling chronically ill patients and their families to contribute to the visit note and diagnostic process and identify and report diagnostic breakdowns using existing EHR data.
- Assess the use and impact of this method on the diagnostic process, measuring safety (incidence/types of PRDBs among chronically ill patients), implementation, and stakeholder experience outcomes.
