EvidenceNOW: Cooperatives

EvidenceNOW established seven regional cooperatives composed of public and private health partnerships and multidisciplinary teams of experts. Each cooperative is recruiting and engaging 250–300 small- and medium-sized primary care practices and providing quality improvement services typically not available to the practices because of their size. The services include onsite practice facilitation and coaching, expert consultation, shared learning collaboratives, and electronic health record support.

Healthy Hearts Northwest consists of Washington, Oregon and Idaho. Healthy Hearts in the Heartland are Wisconsin, Illinois, and Indiana. HealthyHearts NYC is in New York City. Heart of Virginia Healthcare is in Virginia. Heart Health NOW! is in North Carolina. Healthy Hearts of Oklahoma is in Oklahoma. EvidenceNOW Southwest consists of New Mexico and Colorado.The cooperatives span 12 States that deliver health care in a range of metropolitan and rural settings and serve diverse populations. EvidenceNOW will reach Native Americans in Oklahoma, Hispanics in New Mexico and Colorado, medically underserved communities in New York City and the Chicago Tri-State region, and rural communities in the Pacific Northwest, Virginia, and North Carolina. EvidenceNOW will ultimately reach over 5,000 primary care professionals serving approximately 8 million people.
