Not all practices are ready to benefit from practice facilitation. This resource, part of an AHRQ curriculum used to train practice facilitators, outlines criteria and signs that primary care practices are ready to work with facilitators on improving the quality of care they deliver. It contains an initial readiness assessment checklist and guidance on conducting an assessment after 3 months of facilitation.
