This dashboard includes information organized by (1) Patient and Event Characteristics and (2) Intervention Patterns by Patient Activity and Risk Factors (V1.2 only). Selecting one of the options on the top table below will display multiple dashboards with related figures and tables. Please refer to the downloadable Excel file in the Additional Resources box for the aggregate counts used to populate each of the figures and tables.
The Network for Patient Safety Databases (NPSD) does not contain a representative sample of patient safety concerns and cannot be used to calculate the actual incidence or prevalence of patient safety concerns. The reporting of patient safety concerns to the NPSD is voluntary, as is the reporting to Patient Safety Organizations (PSOs) by providers. The NPSD is a summary of certain elements in Hospital Common Formats Events Reports for specific types of patient safety concerns that have been submitted voluntarily by a portion of the AHRQ-listed PSOs.
The total number of reported fall incidents in the NPSD from January 1, 2010, through December 31, 2025, is 450,520 events.
The Falls Dashboard includes a section devoted to reporting single interventions and combinations of interventions using a method called "Frequent Pattern Mining." The Intervention Patterns by Patient Activity and Risk Factors analysis represent data only submitted in Common Formats for Event Reporting-Hospital (CFER-H) V1.2. For each specific subgroup of falls events based on patient injury status and patient residual harm, the most frequent patterns are presented in decreasing order of frequency. Each pattern listed represents a combination of interventions reported in a single event. A single patient event can have multiple interventions indicated and be represented in more than one combination of interventions. Therefore, the counts of all frequent patterns do not add up to the number of all records in the analysis. For brevity, only the 20 most frequent combinations of interventions are displayed.
