The Network for Patient Safety Databases (NPSD) does not contain a representative sample of patient safety concerns and cannot be used to calculate the actual incidence or prevalence of patient safety concerns. The reporting of patient safety concerns to the NPSD is voluntary as is the reporting to Patient Safety Organizations (PSOs) by providers. The NPSD is a summary of certain elements in Common Formats for Event Reporting-Hospital (CFER-H) for specific types of patient safety concerns that have been submitted voluntarily by a portion of the AHRQ-listed PSOs. This dashboard examines all reports on patient safety concerns submitted to the Patient Safety Organization Privacy Protection Center (PSOPPC). The dashboard charts detail Cumulative Number of Reports Submitted by Common Formats Version by Year, Completeness of Reports Submitted by Common Formats Version, Percentage of Total Reports by Common Formats Version, Percentage of Total Reports by Report Type and Percentage of Event Type by Common Formats Version. Selecting one of the options in the top table below will display a related figure and table. This information can also be downloaded as an Excel file from the links in the Additional Resources box.
