Comorbidities associated with hospitalization and progression among adolescents with symptomatic coronavirus disease 2019.
This study’s objective was to identify subgroups likely to benefit from monoclonal antibody and antiviral therapy to treat COVID-19 by evaluating the relationship between comorbidities and hospitalization among US adolescents aged 12-17 with symptomatic coronavirus disease 2019 (COVID-19). The authors used the Pediatric COVID-19 US registry to identify patients who met their inclusion criteria of comorbidities including obesity, chronic kidney disease (CKD), diabetes, immunosuppressive disease or treatment, sickle cell disease (SCD), heart disease, neurologic disease/neurodevelopmental disorders, and pulmonary disease (excluding patients with mild asthma). Out of 1877 total patients included 284 (15%) were hospitalized within 28 days of their COVID-19 diagnosis. The following comorbidities were associated with increased odds of hospitalization: SCD, immunocompromising condition, obesity, diabetes, neurologic disease, and pulmonary disease (excluding mild asthma).

Assessing proximate intermediates between ambient temperature, hospital admissions, and mortality in hemodialysis patients.
This study investigated whether pre-dialysis systolic blood pressure (preSBP) and interdialytic weight gain (IDWG) in individuals with end-stage kidney disease (ESKD) can independently mediate the association between ambient temperature, all-cause hospital admissions (ACHA), and all-cause mortality (ACM). The study population consisted of 1981 ESKD patients receiving hemodialysis treatments at Fresenius Medical Care facilities in Philadelphia County, PA, from 2011 to 2019. Within a time-to-event framework, the authors estimated the association between daily maximum dry-bulb temperature (TMAX) and, as separate models, ACHA and ACM during warmer calendar months. Based on Lag 2- Lag 1 temporal ordering, and a 1 °C increase in daily maximum dry-bulb temperature (TMAX), the authors found an increased hazard of ACHA by 1.4% and ACM 7.5%. Short-term lag exposures to 1 °C increase in temperature predicted mean reductions in IDWG and preSBP by 0.013-0.015% and 0.168-0.229 mmHg.

Association of acute kidney injury with readmissions after hospitalization for acute exacerbation of chronic obstructive pulmonary disease: a population-based study.
This study examined the association between acute kidney injury (AKI) and readmission with hospitalization for acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Retrospective, population-based cohort data was used from the HCUP State Inpatient Databases from seven states (Arkansas, California, Florida, Iowa, Nebraska, New York, and Utah) from 2010 through 2013. A total of 356,990 patients were identified as hospitalized for AECOPD. Median age was 71 years and 41.9% were male. Of those 7% had a concurrent diagnosis of AKI. Patients with AKI were found to have a significantly higher risk of 30-day all-cause readmission compared to those without AKI as well as a significantly higher risk of 90-day all-cause readmission, particularly for non-respiratory reasons. These reasons included sepsis, acute renal failure, and congestive heart failure.

Comparative effectiveness of high-dose versus standard-dose influenza vaccine among patients receiving maintenance hemodialysis.
This study compared outcomes of routine dialysis patients who received either the standard dose vaccine (SDV) for influenza or a high-dose vaccine (HDV). Out of 22,215 influenza patient-seasons among adults 65 years and older, 97.4% received SDV and 2.6% received HDV. There did not seem to be any additional protection provided by the HDV for all-cause mortality or influenza-related outcomes.

The perfect storm: stakeholder perspectives on factors contributing to hospital admissions for patients undergoing maintenance hemodialysis.
This study examined reasons why end stage renal disease (ESRD) patients undergoing hemodialysis experience frequent complications and hospitalizations. Interviews and focus groups were conducted with 21 patients living with ESRD, 10 caregivers, and three focus groups with health care professionals. There were four main themes that came out of the discussions: graft site/catheter/access issues, low resistance, breathing problems, and “The perfect storm”. The goal of these interviews were to promote more research to improve transitional care and care delivery for hemodialysis patients.

Post-hospitalization dialysis facility processes of care and hospital readmissions among hemodialysis patients: a retrospective cohort study.
This study examined the association of post-hospitalization processes of care at hemodialysis facilities with pulmonary edema-related and other readmissions. The study results suggested that some interventions performed at the dialysis facility in the post-hospitalization period may be associated with reduced readmission risk, while others may provide a potential existing means of identifying patients at higher risk for readmissions, to whom such interventions could be efficiently targeted.

Burden and correlates of readmissions related to pulmonary edema in US hemodialysis patients: a cohort study.
Pulmonary edema is prevalent and may be a common cause of hospital readmissions in hemodialysis patients. In this retrospective cohort study, the investigators aimed to estimate the national burden of, and identify correlates of, readmissions related to pulmonary edema among hemodialysis patients. The investigators concluded that readmissions related to pulmonary edema were common in hemodialysis patients. They suggest that interventions aimed at preventing such readmissions could have a substantial impact on readmissions overall, particularly targeted at incident hemodialysis patients with a prior history of heart failure and patients initially admitted for pulmonary edema.

Free and total p-cresol sulfate levels and infectious hospitalizations in hemodialysis patients in CHOICE and HEMO.
P-cresol sulfate (PCS), a prototype protein-bound uremic retention solute, has been shown to exert toxic effects in vitro. The researchers explored the relationship between free and total PCS and indoxyl sulfate (IS) with infection-related hospitalizations (IH) and septicemia in 2 cohorts. Their results suggest an association between higher concentrations of free PCS and infection-related and sepsis-related hospitalizations in hemodialysis patients.

Hemodialysis hospitalizations and readmissions: the effects of payment reform.
The researchers used a retrospective cohort interrupted time-series study design to examine whether the 2004 nephrologist reimbursement reform led to reduced hospitalizations and rehospitalizations. They found no significant change in all-cause hospitalization or rehospitalization and slight reductions in fluid overload hospitalization and rehospitalization following reimbursement reform.

Categories of hospital-associated acute kidney injury: time course of changes in serum creatinine values.
The objective of this study was to categorize hospital-associated acute kidney injury (HA-AKI) based on the timing of minimum and peak inpatient serum creatinine (sCr) and describe the association with inpatient mortality. It concluded that risk of short-term inpatient mortality is associated with AKI, and this risk is attenuated with recovery of kidney function in the hospital.

Effect of intravenous iron use on hospitalizations in patients undergoing hemodialysis: a comparative effectiveness analysis from the DEcIDE-ESRD study.
The researchers studied the association of receipt of intravenous iron with hospitalizations in an incident cohort of hemodialysis patients. They concluded that a higher cumulative dose of intravenous iron may not be associated with increased risk of hospitalizations in hemodialysis patients. While clinical trials are needed, employing higher iron doses to reduce erythropoiesis-stimulating agents does not appear to increase morbidity in routine clinical care.
