A flowchart illustrates how information flows between the consumer, governing body, system hub, organizations, and health care facilities.

  • Consumer to system hub: The consumer experiences an event. The event can be reported locally or reported nationally. The report flows toward the system hub. System staff and multilevel analytic engine are within the system hub. The system hub responds with feedback to the consumer, including public reporting and individual feedback.
  • Health care facilities to and from system hub: the raw report data from the consumer may be immediately forwarded to the local system, flowing toward health care facilities. Health care facilities forward reports to the system hub through root-cause analyses. The system hub sends feedback to health care facilities.
  • Governing body to system hub: Governing bodies send input to the system hub.
  • System hub to and from other systems and organizations: Information flows back and forth between the system hub and other systems and organizations. Other systems include: national systems and organizations; international systems and organizations; State systems and organizations; and local systems and organizations.
