This module builds upon Module 5 which discussed how to build an automatic referral (AR) order set. Expert speakers will cover areas such as how to test the AR order set, prepare for go-live, and developing a data and monitoring plan.

- Staff training.
- Roll-out and reinforcement of automatic referral.
- Evaluation and improvement of automatic referral.
- Extending and spreading automatic referral use.
The target audiences for this session include:
- The CR QI team
- IT team
- Representatives of QI and data analytics teams
- Non-team cardiac clinicians
- Discharge planners
- Care coordinators
- Hospital leadership
It is important to note that the audiences listed above are a suggestion rather than a limitation. All are welcome to watch and review the module.
Upon completion of this module, attendees should be able to:
- Complete testing the AR order set in the EMR.
- Communicate, educate and support go-live.
- Develop a data and feedback monitoring plan
- Troubleshoot common problems that may arise
