SOPS Medical Office Survey

AHRQ sponsored the development of the Surveys on Patient Safety Culture® (SOPS®) Medical Office Survey in response to requests for a survey in this setting. This survey is designed specifically for outpatient medical office providers and staff, and asks for their opinions about the culture of patient safety and healthcare quality in their medical offices.

Medical Office Survey 1.0

Image shows the Topics Covered by the SOPS Medical Office Survey. A composite measure is a grouping of two or more survey items that assess the same area of culture. The 10 composite measures and 38 survey items assessed in the SOPS Medical Office Survey are:; Communication About Error (4 items); Communication Openness (4 items); Office Processes and Standardization (4 items); Organizational Learning (3 items); Overall Perceptions of Patient Safety and Quality (4 items); Owner/ Managing Partner/ Leadership Support for Patient Safety (4 items); Patient Care Tracking/ Followup (4 items); Staff Training (3 items); Teamwork (4 items); Work Pressure and Pace (4 items). Single Item Measures: In addition to the composite measures, single item measures included assess List of Patient Safety and Quality Issues (9 items); Information Exchange With Other Settings (5 items);Screener Question Determining if Respondent is an Owner, Managing Partner, or in a Leadership Position (1 item); Overall Ratings on Quality and Patient Safety (6 items); Background Questions (3 items).
